Effect of Circulating MiR-146a Expression in Type 2 Diabetes Mellitus with Osteo-porosis and Its Correlation with RANKL/RANK/OPG
Objective:To explore the expression and diagnostic value of circulating miR-146a-5p in T2DM patients complicated with osteoporosis(OP)and its correlation with RANKL/RANK/OPG system.Methods:This study recruited 84 T2DM male patients with/without OP(n=54 for T2DM,n=30 for T2DM+OP)in hospital from December 2021 to Feb-ruary 2023.miR-146a expressions in plasma were measured with qRT-PCR.OPG and sRANKL expressions in plasma were measured with ELISA.Results:miR-146a and OPG plasma levels in T2DM+OP group was obviously decreased,while sRANKL level was significantly higher than T2DM group.Univariate logistic regression analysis showed that body mass index,diabetes duration,C-peptide,miR-146a,sRANKL and OPG were all the influencing factors of T2DM pa-tients with osteoporosis.Multivariate binary logistic regression analysis showed that only miR-146a was an independent influencing factor for T2DM complicated with osteoporosis.miR-146a was positively correlated with the expression level of OPG(r=0.4733),and negatively correlated with the expression level of sRANKL(r=-0.4518).Receiver operating characteristic curve(ROC)analysis of miR-146a(AUC=0.920),sRANKL(AUC=0.870)and OPG(AUC=0.841)showed good diagnostic efficacy in ROC analysis of osteoporosis in T2DM.And miR-146a showed the highest diagnostic efficacy for osteoporosis caused by T2DM,with diagnostic sensitivity of 96.30%,specificity of 80.00%and accuracy of 90.48%.Conclusion:miR-146a expression in plasma was decreased in T2DM with OP.The diagnostic efficacy,sensi-tivity and accuracy of miR-146a in the diagnosis of T2DM complicated with OP were better than sRANKL and OPG,and could be used as a biological diagnostic marker for T2DM complicated with OP.
