摘要

目的 应用二维斑点追踪超声心动图(two-dimensional speckle tracking echocardiography,2D-STE)技术评价左心房功能,探讨左心房大小正常的原发性高血压(高血压)患者心电图P波增宽与左心房功能之间的潜在关联.方法 收集广州市第一人民医院左心房大小正常的高血压患者60例,按心电图P波是否增宽分成心电图P波正常(normal P-wave duration,NP)组(n=20)与心电图P波增宽(widened P-wave duration,WP)组(WP组,n=40).所有入组患者行心电图、常规超声心动图及2D-STE检查.应用2D-STE获取左心房容积及应变参数.比较两组患者上述指标间的差异.结果 两组患者一般资料比较,差异无统计学意义(P>0.05).两组患者的左心房容积参数及左心房射血分数[左心房最大容积(maximum left atrial volume,LAVmax)、左心房最小容积(minimum left atrial volume,LAVmin)、左心房收缩前容积(left atrial presystolic volume,LAVprea)、左心房最大容积指数(maximum left atrial volume index,LAVImax)、左心房总射血分数(total ejection fraction,TEF)、左心房主动排空分数(active emptying fraction,AEF)、左心房被动排空分数(passive emptying fraction,PEF)]比较,差异无统计学意义(P>0.05).两组患者左心房应变参数——左心房收缩期应变(LASct)比较,差异无统计学意义(P>0.05);与NP组比较,WP组左心房整体纵向应变的储存期应变(LASr)及左心房通道期应变(LAScd)降低,差异有统计学意义(P<0.05).结论 2D-STE能检测出左心房大小正常、P波增宽的高血压患者的左心房存储功能和管道功能降低,提示心电图P波增宽可能与早期左心房功能损害存在联系,2D-STE技术可为揭示高血压患者心电图P波增宽与早期左心房功能障碍之间关联性提供依据.

Abstract

Objectives To evaluate the left atrial function using two-dimensional speckle tracking echocardiography(2D-STE)and explore the potential association between widened P-wave duration(WP)on electrocardiogram(ECG)and left atrial function in hypertensive patients with normal left atrial size.Methods Sixty hypertensive patients with normal left atrial size from the Guangzhou First People's Hospital were collected.They were divided into two groups based on whether P-wave widened was present in the ECG:normal P-wave duration(NP)group(NP group,n=20)and WP group(n=40).All the patients underwent ECG,conventional echocardiography and 2D-STE examination.2D-STE was used to obtain left atrial volume and strain parameters.Differences between the two groups were compared.Results There were no significant differences in general data between the two groups(P>0.05).There were no significant differ-ences in left atrial volume parameters and left atrial ejection fraction[maximum left atrial volume(LAVmax),minimum left atrial volume(LAVmin),left atrial presystolic volume(LAVprea),maximum left atrial volume index(LAVImax),total ejection fraction(TEF),active emptying fraction(AEF),passive emptying fraction(PEF)]between the two groups(P>0.05).There was no significant difference in left atrial strain contraction(LASct)parameter between the two groups(P>0.05).Compared with NP group,left atrial longitudinal strain during reservoir phase(LASr)and left atrial strain during conduit phase(LAScd)were decreased in WP group,and the differences were statistically sig-nificant(P<0.05).Conclusions 2D-STE can detect decreased reservoir and conduit function of the left atrium in hy-pertensive patients with normal left atrial size and WP on ECG,suggesting a possible association between WP on ECG and early left atrial dysfunction.2D-STE technology may provide evidence for the relationship between WP on ECG and early left atrial dysfunction in hypertensive patients.
