Establishment and application of perioperative evaluation criteria for tranexamic acid in hip replacement surgery
Objective To establish a comprehensive evaluation standard for the perioperative use of tranexamic acid injection in hip replacement surgery, and use the weighted TOPSIS method for rationality evaluation to provide a basis for rational drug use. Methods Based on the package inserts of tranexamic acid, specialized guide-lines,and relevant literatures,an evaluation standard for tranexamic acid injection during the perioperative period of hip replacement surgery was established. The weighted TOPSIS method was used to comprehensively evaluate 69 medical records of tranexamic acid injection used during the perioperative period of hip replacement surgery in the First People's Hospital of Xianyang from January to December 2023 using multiple indicators. Results Among 69 pa-tients,8.70%had completely reasonable medication records,57.97%had basic rational medication records, and 33.33%had cases of irrational medication. The main unreasonable issues were unreasonable timing and dosage of pre-operative use of tranexamic acid;the timing of sequential use of anticoagulants after tranexamic acid was unreasonable, and the duration of sequential anticoagulation therapy was unreasonable. Conclusion The use of tranexamic acid in-jection during the perioperative period of hip replacement surgery in our hospital is relatively reasonable, but there are still some shortcomings.Clinical attention should be paid to the timing of administration.
