Effect of Electroacupuncture at Lianquan(RN23)on EMG Action Potential of Mylohyoid Muscle and Expression of c-Fos in Rats with Dysphagia
Objective To explore the possible mechanism of electroacupuncture at Lianquan(RN23)in improving swallowing disorder after stroke.Methods A total of 54 male SD rats were randomly divided into Lianquan group,Neiguan group,and blank group with 18 rats in each group(n=12 in each group for physiological experiments,n=6 in each group for immunofluorescence stain).After administering anesthesia,electromyography electrodes were implanted into the mandibular hyoid muscle of rats in each group along the direction of muscle fibers for physiological experiments.To induce swallowing,a microinjection pump was utilized to deliver 3 μL/s of double-distilled water into the throat area of rats over a duration of 30 seconds.electroacupuncture(2 Hz,2 mA,2 mscontinuous wave)was applied to Lianquan(RN23)or left Neiguan(PC6)for 1 min.Rats in the blank group were not given electroacupuncture interventions.electroacupuncture(2 Hz,2 mA,2 mscontinuous wave)was applied to the immunofluorescence stain rats for 20 min.Rats in the blank group were not given electroacupuncture interventions.Swallowing activity(latency of first swallowing,number of swallowing movements)and electromyoelectric action potentials(amplitude,frequency)of mylohyoid muscle were recorded by physiological signal acquisition system in rats.The expression of c-Fos in the nucleus ambiguus region was detected by immunofluorescence.Results Compared with before electroacupuncture,after electroacupuncture and after sham damage of bilateral nucleus ambiguus,the latency of first swallowing in Lianquan group were significantly shortened(P<0.01),the frequency of swallowing movement were increased(P<0.01),and the amplitude and frequency of myohyoid muscle were significantly increased(P<0.01).There was no significant change in the data of Neiguan group(P>0.05).After chemical damage of bilateral nucleus ambiguus,the latency of first swallowing were significantly prolonged(P<0.01),the number of swallowing movements were significantly decreased(P<0.01),and the amplitude and frequency of myohyoid muscle were significantly decreased(P<0.01).Compared with the blank group,after electroacupuncture and after sham damage of bilateral nucleus ambiguus,the latency of first swallowing in RN23 group were significantly shortened(P<0.01),the frequency of swallowing movement were increased(P<0.01),and the amplitude and frequency of myohyoid muscle were significantly increased(P<0.01).There was no significant change in the data of Neiguan group(P>0.05).After chemical damage of bilateral nucleus ambiguus,the latency of first swallowing were significantly prolonged(P<0.01),the number of swallowing movements were significantly decreased(P<0.01),and the amplitude and frequency of myohyoid muscle were significantly decreased(P<0.01).Compared with the blank group,the number of c-Fos positive cells in the nucleus ambiguus region of Lianquan group and Neiguan group were signifcantly increased(P<0.01 or P<0.05).Compared with Neiguan group,the number of c-Fos positive cells in Lianquan group were significantly increased(P<0.01).Conclusion Electroacupuncture administered at the Lianquan(RN23)has the potential to enhance the regulation of swallowing function and stimulate swallowing activity Liangquan in cases where there are normal functioning of the nucleus ambiguus region.This improvement may be attributed to an enhancement in myoelectric action potential expression in the mandibular hyoid muscle and activation of c-Fos neurons within the nucleus ambiguus region.However,further investigation is required to determine which receptor within the nucleus ambiguus region plays a role in regulating swallowing activity.
