摘要

本文报告1例双侧肾结石合并长期泌尿系感染患者的诊治方案.患者接受智能控压经尿道输尿管软镜碎石取石术(intelligent pressure-controlled retrograde intrarenal surgery,IPC-RIRS),手术过程顺利,术后感染好转,术后2 d恢复平稳,拔除尿管出院.术后1个月,复查尿路平片双侧肾未见≥4 mm残石,顺利拔除双J管.术中及术后未出现出血、发热、肾绞痛、深静脉血栓等并发症.肾结石合并感染行输尿管软镜手术具有较高的手术风险,采用前端可弯输尿管测压导引鞘的IPC-RIRS可有效控制腔内压力,且能够对泌尿系感染进行安全地腔内灌洗.

Abstract

This article presents the diagnosis and treatment of a patient with bilateral kidney stones accompanied by a long-term urinary infection.The patient underwent intelligent pressure-controlled retrograde intrarenal surgery(IPC-RIRS),during which the operation proceeded successfully,the infection improved,and the patient was discharged from the hospital with a smooth recovery,and the urinary catheter was removed 2 days after the operation.Kidney ureter bladder X-rays were reviewed 1 month after the operation,revealing no stones ≥4 mm in either kidney,and the double J-tubes were removed without any issues.There were no instances of bleeding,fever,renal colic,deep vein thrombosis,or other complications during or after the operation.Retrograde intrarenal surgery for kidney stones with infection carries a high surgical risk.IPC-RIRS with a tip-bendable pressure measuring ureteral access sheathenables effective intracavitary pressure control and is capable of safe intracavitary irrigation for the treatment of urinary tract infections.
