生殖静脉修补肾盂输尿管成形缺损1例报告

摘要
肾盂输尿管连接部狭窄或单纯输尿管狭窄是临床常见病例,其病因较多且较为复杂.目前输尿管狭窄修复重建时应用最广泛的自体组织包括舌黏膜、颊黏膜、回肠、阑尾以及腹膜等.在应用自体组织修复病变输尿管时,应评估输尿管病变的病因、位置、长度以及自体组织移植物的条件,结合术者经验,进而制订个体化的手术方案.自体组织输尿管修复重建手术应做到输尿管替代材料血供充足,无张力、不漏水吻合.本文介绍1例在狭窄修复重建中遇到了原肾盂材料不够的棘手问题,采取患者生殖静脉制成补片修复缺损后,经1年随访,患者无不良反应,效果良好的病例.
Abstract
Ureteropelvic junction stenosis or simple ureteral stricture is a common clinical case with many and complex causes.Currently,the most widely used autologous tissues in ureteral stricture repair include tongue mucosa,buccal mucosa,ileum,appendix,and peritoneum.When using autologous tissues to repair diseased ureters,the cause,location,length of the ureteral lesions and the conditions of the autologous tissue graft should be evaluated,and an individualized surgical plan should be developed based on the surgeon's experience.Autologous tissue ureteral repair and reconstruction should be performed with sufficient blood supply of ureteral replacement materials,and the anastomosis should be tension-free and leak-free.This article reports a case in which the original renal pelvis material was insufficient during repair and reconstruction of stricture.The patient's genital vein was used to make a patch to repair the ureteral defect.After 1 year of follow-up,good results were obtained without complications.
