Clinical application value of fetal heart measurement parameter Z-score in screening fetal heart structural malformations
Aim To explore the clinical application value of fetal heart measurement parameter Z-score in screening fetal heart structural malformations.Methods A total of 93 fetuses with heart structural malformations were enrolled.According to different malformations,fetuses were divided into right heart dysplasia group(n=48)and left heart dysplasia group(n=45).A total of 45 normal fetuses during the same period were enrolled as control group.Fetal biological growth parameters such as biparietal diameter(BPD),femur length(FL)and gestational age(GA)were selected as independent variables,and heart measurement pa-rameters such as diameters of aortic valve(AO),ascending aorta(AAO),pulmonary valve(PA),left pulmonary artery(LPA)and right pulmonary artery(RPA)were selected as dependent variables,to conduct heart measurement parameter Z-score.The differ-ences in fetal Z-score among all groups were compared.The diagnostic efficiency of different heart measurement parameter Z-score for heart structural malformations was analyzed and compared by ROC curves.Results Compared with control group,PA,LPA and RPA-Z were decreased,while AO and AO-Z were increased in right heart dysplasia group(P<0.05),PA,LPA and RPA-Z were in-creased,while AO and AO-Z were decreased in left heart dysplasia group(P<0.05).Compared with left heart dysplasia group,PA,LPA and RPA-Z were decreased,while AO and AAO-Z were increased in right heart dysplasia group(P<0.05).ROC curve characteristics showed that Z-scores of AO,AAO,PA,LPA and RPA were all of diagnostic efficiency for fetal heart structural malfor-mations(P<0.05).The diagnostic value of AO-Z-score was the highest for left heart dysplasia,while diagnostic value of PA-Z-score was the highest for right heart dysplasia(P<0.05).Conclusion Ultrasound Z-scores of fetal aorta and pulmonary artery can e-valuate the growth and development of fetal heart structure,which can provide clinical guidance for prenatal screening of fetal heart structure malformations.
