Relationship between expression of Copeptin,NLR,SIG and PSS score in patients with acute organophosphorus poisoning and its predictive value of curative effect
Objective:To investigate the relationship between the expression of Copeptin,neutrophil-to-lymphocyte ratio(NLR),strong ion gap(SIG)and poisoning severity score(PSS)in patients with acute organophosphorus pesticide poison-ing(AOPP),and its predictive value of efficacy.Methods:From February 2018 to February 2021,68 patients with AOPP in our hospital were divided into an effective group(n=53)and an ineffective group(n=15)according to the clinical effect after 10 days of conventional treatment.The general data,serum Copeptin,NLR,SIG levels and PSS scores before and after treatment were compared between the two groups,and the relationship between serum Copeptin,NLR,SIG levels and PSS scores in AOPP patients before and after treatment was analyzed,and the predictive value of each index was discussed.Results:The time from poisoning to hospital admission in the ineffective group was longer,the degree of disease,the dose of poison,AST,ALT,LDH,and CK levels were higher,and the ChE level was lower than those in the effective group(P<0.05).After 10 days of treatment,serum Copeptin,NLR,SIG levels and PSS scores were higher in the ineffective group than those in the effective group(P<0.05).There was a positive correlation between serum Copeptin,NLR,SIG levels and PSS scores in AOPP patients before treatment and after 10 days of treatment.The high expression of serum Copeptin,NLR and SIG before treatment and 10 days after treatment were independent risk factors for the effect of AOPP treatment(P<0.05).The levels of serum Copeptin,NLR and SIG before treatment predicted the ineffectiveness of AOPP treatment and the area under the curve(AUC)was 0.820,0.692,and 0.758,respectively.The AUC of the joint prediction of inde-xes was the largest,which was 0.908,the best prediction sensitivity was 93.33%,and the best prediction specificity was 81.13%.Conclusion:Serum Copeptin,NLR and SIG can be used as biological indicators to evaluate the severity of poison-ing and predict the curative effect in patients with AOPP.
