CBA方法检测2型糖尿病患者血清细胞因子的临床分析

目的 分析CBA方法检测2型糖尿病(T2DM)患者在不同病变时期血清中Th1、Th2型细胞因子的水平.方法 选取2022年12月至2023年6月于菏泽市中医医院肾病科及内分泌科确诊的T2DM患者60例作为研究对象,将患者根据持续性白蛋白尿和(或)肾小球滤过率(glomerular filtration rate,GFR)进行性下降分为:(1)糖尿病组(DM组):尿微量白蛋白阴性;(2)糖尿病肾病组(DN组):尿微量白蛋白阳性或肾小球滤过率下降;(3)同期选择21例健康者作为对照组(HC组).利用微量样本多重蛋白定量技术(cyto-metric bead array,CBA),采用BD公司的BD CBA Th1/Th2/Th17细胞因子试剂盒,检测研究对象血清中IFN-γ、TNF、IL-2、IL-4、IL-6、IL-10水平.结果 (1)利用CBA检测方法获得T2DM患者在不同病变时期以及健康对照者Th1、Th2型细胞因子的含量范围.(2)Th1型细胞因子中血清IFN-γ水平在HC组、DM组、DN组中呈现先升高后降低的趋势,差异具有统计学意义(P<0.05);Th2型细胞因子中TNF、IL-2、IL-4和IL-6水平均随病情具有先升高后下降的趋势;IL-10水平则呈现逐渐增高的趋势.结论 (1)首次利用CBA检测方法得到T2DM不同病变时期以及健康人群血清中Th1、Th2型细胞因子的含量范围.(2)明确Th1、Th2型细胞因子在T2DM病程中的变化趋势.(3)Th1、Th2型细胞因子整体水平动态变化可以作为检测病情进展的重要指标.(4)IL-2、IL-4和TNF 3个指标与T2DM早期阶段密切相关,提示其中任何一项指标都可以作为T2DM早期阶段的主要检测指标;而IL-10则是糖尿病肾病阶段密切相关指标,提示其可以作为该阶段病情或者药物疗效的主要检测指标.
Objective Using CBA method,to detect the serum levels of Th1,Th2 cytokines in patients with T2DM at different pathological stages.Methods A total of 60 T2DM patients diagnosed by the division of nephrology and endocrinology of Heze hospital of traditional Chinese medicine were selected in this study.Study participants were recruited from December 2022 to June 2023.The groups were divided according to continuity and(or)albuminuria in patients with glomerular filtration rate.Diabetes group(DM group):the urine trace al-bumin is negative;Diabetic nephropathy group(DN group):urine microalbumin positive or glomerular filtration rate decreased;21 healthy cases from the same area in same period were selected as the control group(HC group).The serum levels of IFN-γ,TNF,IL-2,IL-4,IL-6,IL-10 were determined using the BD CBA Th1/Th2/Th17 cytokine kit.Results(1)The serum concentrations range of Th1,Th2 cytokines in pa-tients with T2DM at different stages of disease and in healthy controls was obtained by using CBA method.(2)The level of serum IFN-γ in Th1 type cytokines showed a trend of increasing first and then decreasing in HC group,DM group and DN group(P<0.05);the serum levels of TNF,IL-2,IL-4 and IL-6 had a tendency to increase first and then decrease with the condition;IL-10 level shows a trend of increasing gradually.Con-clusion(1)For the first time,CBA detection method was used to obtain the content ranges of Th1,Th2 cy-tokines in serum of healthy people and patients at different pathological stages of T2DM.(2)The change trend of Th1,Th2 cytokines in the course of T2DM was clarified.(3)Dynamic changes in the overall levels of Th1,Th2 cytokines can be used as important indicators to detect disease progression.(4)Three indicators of IL-2,IL-4 and TNF are closely related to the early stage of T2DM,suggesting that any one of these indicators can be used as the main detection indicator in the early stage of T2DM.However,IL-10 are closely related indicators in the stage of diabetic nephropathy,suggesting that either of these indicators can be used as the main detection in-dicators of the disease or drug efficacy at this stage.
