3D Printing Guide Plate Combined with Arthroscopy Assisted High Tibial Osteotomy For Osteoarthritis
Objective To explore the clinical efficacy of preoperative 3D printing patient-specific instru-mentation(PSI)combined with intraoperative arthroscoy assisted high tibial osteotomy(HTO)in the treatment of knee joint unilateral medial compartment osteoarthritis.Method 68 patients with unilateral medial compart-ment osteoarthritis of knee joint into our hospital from February 2020 to February 2022 were prospectively in-cluded,meeting the indications of HTO surgery.They were randomly divided into control group and observation group,with 34 patients in each group.The control group used arthroscopy assisted HTO,the observation group used 3D printing PSI before surgery on the basis of the control group.Then surgical indicators(surgical time,intraoperative bleeding volume,and postoperative drainage time),pain visual analogue scale(VAS)score(1 day,7 days,1 month,and 3 months after surgery),Lysholm score,knee joint range of motion(ROM),tibiofemoral angle(before surgery,1 month,3 months,6 months,and 12 months after surgery),and complica-tions.Results 31 patients in the control group and 32 patients in the observation group were followed up for 12 months,the general clinical datas between the two groups were comparable(P>0.05).The surgical time in the observation group was significantly shorter than that in the control group(P<0.05),there were no differences of bleeding volume and drainage time(P>0.05).There were no differences of pain VAS score on the 1st and 7th postoperative days(P>0.05),while the VAS score of the observation group at the 1st and 3rd postoperative months were significantly lower than those of the control group(P<0.05).There were no differences of Lysholm score,knee ROM,and tibiofemoral angle before and 1 month after surgery(P>0.05),but Lysholm score and knee ROM of the observation group were significantly higher at 3,6,and 12 months after surgery,while tibiofemoral angle was significantly lower than the control group(P<0.05).There were no differences of the complication rate between the two groups during follow-up(P>0.05).Conclusion Preoperative 3D printing PSI can furtherly improve the surgical accuracy of arthroscopy assisted HTO for knee joint single medial compartment osteoarthritis,shorten surgical time,reduce postoperative pain,and improve postoperative joint function,which has important application potential.
