Analysis of Adverse Reactions and Events of Metformin Associated Lactic Acidosis
Objective To explore the regularity and characteristics of adverse drug reactions and events(ADR/ADE)in metformin associated lactic acidosis(MALA)in order to improve the safety of clinical drug use.Methods The data of China knowledge Network,Wanfang and VIP were searched until Sep 30,2023,and the published cases of adverse drug reactions and events related to MALA in China were analyzed retrospectiv-ely.Results A total of 40 cases of MALA adverse reactions were collected,including 20 males and 20 females.The average age is 60.2 years old,and 57.5%are over 60 years old.27 patients(67.5%)had risk factors for inducing MALA,of which 14(35.5%)were given the maximum dose of the instructions,and 13(32.5%)considered at least one risk factor for adverse reactions,including drinking behavior,history of coronary heart disease,cardiac insufficiency or myocardial infarction,history of cerebral infarction,complicated with renal in-sufficiency,complicated with liver insufficiency,infection induction,etc.After treatment,31 patients improved and 9 patients died,there is a certain relationship between the related laboratory data and the prognosis of pa-tients with MALA.Conclusion There was no significant gender difference in the occurrence of MALA,and the risk of MALA was higher in elderly patients.The occurrence of MALA may be related to the risk factors of merger.For patients at higher risk of MALA,such as overdose medication,hepatorenal insufficiency,acute gastrointestinal disease,severe hypoxia,heart failure,severe infection,surgery,alcohol consumption and con-trast agent use,avoid or stop using metformin.For the elderly and patients with mild and moderate renal insuf-ficiency,metformin should be used to closely monitor the renal function of the patients,strengthen the follow-up and,if necessary,monitor the level of blood lactic acid to adjust or stop the drug in time.
