2型糖尿病视网膜病变患者血清中Asprosin水平变化

目的 测定糖尿病视网膜病变(diabetic retinopathy,DR)患者血清中脂肪因子白脂素(Asprosin)、超氧化物歧化酶(superoxide dismutase,SOD)、丙二醛(malondialdehyde,MDA)及血管内皮生长因子(vascular endothelial growth factor,VEGF)水平,分析4种因子与DR及氧化应激的相关性。方法 将2022年11月至2023年10月在内蒙古自治区人民医院眼科住院的2型糖尿病(type 2 diabetes mellitus,T2DM)患者139例纳入研究。将患者结合DR分期标准分为3组:增殖期糖尿病视网膜病变(proliferative diabetic retinopathy,PDR)组 39 例,非增殖期糖尿病视网膜病变(non-proliferative diabetic retinopathy,NPDR)组47例,糖尿病无视网膜病变(non-diabetic retinopathy,NDR)组53例,单纯白内障患者61例为对照组(NC组)。采用酶联免疫吸附法测定各组血清中Asprosin、SOD、MDA及VEGF水平,同时,收集患者性别、年龄、体质量、身高、BMI、糖尿病病程、空腹血糖(fasting plasma glucose,FPG)、空腹C肽、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、空腹胰岛素(fasting insulin,FIN)、低密度脂蛋白(low-density lipopro-tein cholesterol,LDL-C)、甘油三酯(triglyceride,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白(high-density lipoprotein cholesterol,HDL-C)等指标,计算胰岛素抵抗指数(homeostatic model assessment of insulin resistance,HOMA-IR),将以上数据进行统计学分析。结果 PDR组血清Asprosin、MDA及VEGF水平高于NPDR组及NDR组,且均高于NC组,两两组间差异具有统计学意义;相关性分析显示:糖尿病病程、HbA1c、FIN、FPG、HOMA-IR、空腹 C 肽、HDL-C、LDL-C、HS-CRP 与 Asprosin 具有相关性。MDA与糖尿病病程、HbA1c、FIN、FPG、HOMA-IR、空腹C肽、LDL-C具有相关性;VEGF与HbA1c、FIN、FPG、HOMA-IR、HDL-C、LDL-C 及 HS-CRP 具有相关性;SOD 与糖尿病病程、HbA1c、FPG、FIN、HOMA-IR、HDL-C、LDL-C及HS-CRP具有相关性。4种因子与DR具有相关性,Asprosin预测DR的ROC曲线下面积为0。903,SOD曲线下面积为0。923,MDA曲线下面积为0。902,VEGF曲线下面积为0。899。结论 Asprosin、SOD、MDA及VEGF均与DR具有相关性;血清中Asprosin、MDA及VEGF的水平随着DR严重程度加重而上升;Asprosin、SOD、MDA及VEGF对于DR具有一定预测作用,其中Asprosin较MDA、VEGF两种因子更好,当其约登指数最大时,其特异度和敏感度分别为73。68%和90。70%,As-prosin 或可作为一种新型脂肪因子预测DR的发生。
To Investigate the Changes of Serum Asprosin Levels in Diabetic Retinopathy Patients at Different Stages
Objective To determine the serum levels of adipose factors such as Asprosin,Superoxide Dis-mutase(SOD),malondialdehyde(MDA)and vascular endothelial growth factor(VEGF)in patients with dia-betes retinopathy(DR),and to analyze the correlation between the four factors and DR and oxidative stress.Methods 139 patients with Type 2 diabetes Mellitus(T2DM)hospitalized in the ophthalmology department of the People's Hospital of Inner Mongolia Autonomous Region from November 2022 to October 2023 were select-ed for the study;The patients were divided into three groups according to DR staging criteria:proliferative dia-betes retinopathy(PDR)group(39 cases),non proliferative diabetes retinopathy(NPDR)group(47 cases),diabetes non diabetic retinopathy(NDR)group(53 cases),and simple cataract patients(61 cases)as the con-trol group.The serum levels of Asprosin,SOD,MDA and VEGF were measured by enzyme-linked im-munosorbent assay.At the same time,the patient's gender,age,weight,height,BMI,course of diabetes,fasting plasma glucose(FPG),fasting C-peptide,glycosylated hemoglobin(HbA1c),fasting insulin(FIN),low-density lipoprotein cholesterol(LDL-C),triglyceride(TG),and total Total cholesterol(TC),high den-sity lipoprotein cholesterol(HDL-C)and other indicators,calculate the Homeostatic model assessment of in-sulin resistance(HOMA-IR),and statistically analyze the above data.Results The serum levels of Asposin,MDA,and VEGF in PDR patients were higher than those in NPDR group and NDR group,all higher than those in NC group.The difference between the two groups was statistically significant;Correlation analysis showed that the course of diabetes HbA1c、FIN、FPG、HOMA-IR、Fasting C-peptide,HDL-C,LDL-C,HS-CRP are correlated with Asposin.MDA and the course of diabetes HbA1c、FIN、FPG、HOMA-IR、There is a correlation between fasting C-peptide and LDL-C;VEGF is correlated with HbA1c,FIN,FPG,HOMA-IR,HDL-C,LDL-C,and HS-CRP;SOD was associated with the course of diabetes,HbA1c,FPG,FIN,HOMA-IR,HDL-C,LDL-C and HS-CRP.Four factors are correlated with DR.Asposin predicts that the area under the ROC curve of DR is 0.903,the area under the SOD curve is 0.923,the area under the MDA curve is 0.902,and the area under the VEGF curve is 0.899.Conclusion Asposin,SOD,MDA,and VEGF are all correlated with DR;The levels of Asposin,MDA,and VEGF in serum increase with the severity of DR;Asposin,SOD,MDA,and VEGF have a certain predictive effect on DR,among which Asposin is better than MDA and VEGF.When its Jordan index is highest,its specificity and sensitivity are 73.68%and 90.70%,respectively.Asposin may be used as a new type of adipose factor to predict the occurrence of DR.
