不同类型分水岭梗死与血管狭窄的相关因素分析

摘要
目的 本研究通过对不同类型分水岭梗死(WSI)者入院后的病历资料统计分析,探讨颈内动脉血管狭窄程度在不同类型WSI间是否存在差异.方法 选取于哈尔滨医科大学附属第一医院神经内科住院治疗的70例WSI患者,将WSI患者根据新发梗死部位分为皮质型、皮质下型和混合型.皮质型分为皮质前型,皮质后型.皮质下型分为部分型和融合型.分析比较不同梗死类型之间是否存在颈内动脉及其他血管重度狭窄或闭塞.结果 WSI皮质型、皮质下型、混合型3组间同侧ICA颅内段有无重度狭窄近闭塞(P=0.028)差异有统计学意义.皮质前型、皮质后型、皮质前后型3组间同侧大脑前动脉狭窄差异有统计学意义(P=0.031).皮质下型两组间同侧ICA颅内段、颅外段及MCA狭窄分别有统计学意义(P=0.003,P=0.027,P=0.010).结论 分水岭各亚型间血管狭窄程度与部位存在差异,融合型皮质下WSI更易存在颈内动脉严重病变.
Abstract
Objective In this study,patients with different kinds of acute watershed infarction(WSI)were statistically analyzed after admission to explore whether the stenosis degree of ICA vessel stenosis is different.Methods 70 patients with WSI were selected for routine inpatient evaluation.We divided the patients into cortical watershed infarction(CWI),internal watershed infarction(IWI)and mixed watershed infarction(MWSI).CWI was divided into anterior watershed infarction,posterior watershed infarction and both of them,IWI was divided into partial IWI(P-IWI),and Confluent IWI(C-IWI).Analysis the clinical datas of patients with different types.Results There is a significant difference among the three groups in whether there is severe stenosis or near occlusion in the intracranial segment of ICA(P=0.028).There is a significant difference in anterior cerebral artery stenosis among the three groups of cortical watershed infarction(P=0.031).Ipsilateral ICA intracranial and extracranial segment,and MCA stenosis are significantly different between the two groups of IWI(P=0.003,P=0.027,P=0.010,respectively).Conclusions There are differences in the degree and location of vascular stenosis among the subtypes of WSI,and the severe lesions of internal carotid artery are more likely to occur in patients with the confluent internal WSI.
