CT纹理参数峰度、熵值联合预测食管癌患者术前T分期的价值

摘要
目的 探讨CT纹理参数峰度、熵值联合评估食管癌患者术前T分期的价值.方法 选取2022年2月～2023年6月在医院接受手术治疗的食管癌患者70例,所有患者均接受CT检查,且在检查1周内实施手术,并对T分期结果进行判断,根据结果分为早期组与中晚期组,设计基线资料调查表,详细统计两组患者的基线资料与CT纹理参数、熵值并比较,重点分析CT纹理参数峰度、熵值联合评估食管癌T分期的价值.结果 手术病理结果显示,70例食管癌患者T1期12例,T2期18例,T3期30例,T4期10例;其中早期组30例,占比42.86％(30/70),中晚期组40例,占比57.14％(40/70);中晚期组患者的CT纹理参数峰度、熵值均比早期组高(P＜0.05),组间性别、年龄、肿瘤直径、病理分型、病灶部位、吸烟史、合并慢性病及CT纹理参数均值、偏度比较,无统计学差异(P＞0.05);绘制ROC曲线发现,食管癌患者T分期使用CT纹理参数峰度、熵值评估的AUC均＞0.80,评估价值理想,且以联合评估价值最佳.结论 CT纹理参数峰度、熵值评估食管癌患者T分期价值理想,CT纹理参数峰度、熵值过表达提示食管癌T分期较高.
Abstract
Objective To investigate the value of CT texture parameters kurtosis and entropy in e-valuating preoperative T staging of patients with esophageal cancer.Methods A total of 70 pa-tients with esophageal cancer who received surgical treatment in hospital from February 2022 to June 2023 were selected.All patients received CT examination and underwent surgery within 1 week of examination.The T-stage results were evaluated and divided into early group and middle and late group according to the results,and a baseline data questionnaire was designed.The base-line data of the two groups of patients were analyzed in detail and compared with CT texture pa-rameters and entropy,focusing on the value of the combined evaluation of CT texture parameters kurtosis and entropy in the T staging of esophageal cancer.Results The pathological results showed that there were 12 cases of T1 stage,18 cases of T2 stage,30 cases of T3 stage and 10 cases of T4 stage.There were 30 cases in the early group,accounting for 42.86％(30/70),40 cases in the middle and late group,accounting for 57.14％(40/70).The kurtosis and entropy of CT texture parameters in the middle and advanced group were higher than those in the early group(P＜0.05),but there were no statistical differences between groups in gender,age,tumor diameter,pathological type,lesion location,smoking history,combined chronic diseases,mean value and skewness of CT texture parameters(P＞0.05).The ROC curve showed that the AUC evaluated by CT texture parameters kurtosis and entropy were all＞0.80 in T stage of patients with esophageal cancer,and the evaluation value was ideal,and the combined evaluation value was the best.Conclusion The value of CT texture parameter kurtosis and entropy in evaluating T staging of patients with esophageal cancer is ideal,and the overexpression of CT texture pa-rameter kurtosis and entropy indicates higher T staging of esophageal cancer.
