摘要
非酒精性脂肪肝(Nonalcoholic fatty liver disease,NAFLD)是一种常见的临床代谢综合征,其发病率近年来迅速增加.目前西医治疗上尚无理想的特效药,且有一定副作用,但若不及时干预,不仅会影响肝胆系统,甚则发展为肝癌.而中药调理是中医治疗NAFLD最常用的特色方法之一,笔者根据长期跟诊以及对医案资料的学习,分析总结了陈涤平教授对NAFLD的诊疗思路.陈涤平教授从事中医脾胃病临床工作及治未病研究工作近40年,注重"未病状态"辨识,应用分期论治NAFLD患者一例,疗效显著.陈师认为脾虚生湿乃是本病病机关键所在,本病病位在肝脏,并与脾脏密切相关,应在"治未病"理论指导下进行分期治疗,初期以"清热燥湿"为主,中期以"活血化瘀"为主,后期以"疏肝调气"为主,即治疗本病的"三部曲",在临床上颇具疗效,为NAFLD患者的辨证施治提供一定参考.
Abstract
Nonalcoholic fatty liver disease(NAFLD)is a common clinical metabolic syndrome,and its incidence has increased rapidly in recent years.At present,there is no ideal specific medi-cine in Western medicine treatment,and it has certain side effects,but if not timely intervention,it will not only affect the liver and gallbladder system,but even develop into liver cancer.And traditional Chinese medicine conditioning is one of the most commonly used characteristics of tra-ditional Chinese medicine in the treatment of NAFLD.The author analyzes and summarizes Pro-fessor Chen Diping's diagnosis and treatment ideas for NAFLD based on long-term follow-up and the study of medical records.Professor Chen Diping has been engaged in the clinical work and treatment of spleen and stomach diseases in traditional Chinese medicine for nearly 40 years.He pays attention to the identification of"undiseased state"and applies the staging theory to treat a case of NAFLD,and the effect is remarkable.Chen Shi believes that spleen deficiency and damp-ness generation is the key to the pathogenesis of the disease.The disease is located in the liver and is closely related to the spleen.The treatment should be carried out in stages under the guid-ance of the theory of treating the disease before it".The initial stage is mainly"clearing heat and dryness",the middle stage is mainly"promoting blood circulation and removing blood stasis",and the later stage is mainly"relieving the liver and regulating qi",that is,the"trilogy"of the treatment of this disease.It has considerable clinical efficacy and provides a certain reference for the syndrome differentiation and treatment of NAFLD patients.
