PLA2R、THSD7A在肾病综合征患者的表达及与治疗效果的关系分析

摘要
目的 观察抗磷脂酶A2受体(PLA2R)和1型血小板反应蛋白7A域(THSD7A)在肾病综合征患者肾组织中的表达,并分析其与治疗效果的关系.方法 选取医院2020年5月至2022年4月收治的98例肾病综合征患者,采用免疫发光法检测患者肾组织中的PLA2R和THSD7A表达情况;给予糖皮质激素联合免疫抑制剂治疗2个月,根据PLA2R和THSD7A检测结果将患者分为PLA2R阴性组及阳性组、THSD7A阴性组及阳性组,比较PLA2R/THSD7A阳性及阴性组治疗3、6、12个月后的肾功能指标、治疗总有效率及不良反应.结果 PLA2R阳性表达率为61.22％(60/98),阴性表达率为 38.78％(38/98);THSD7A 阳性表达率为 33.67％(33/98),阴性表达率为66.33％(65/98).各阴/阳性组性别、年龄、病程、合并糖尿病、高血压、高脂血比较无差异(P＞0.05).PLA2R/THSD7A阳性组治疗后3、6、12个月的24 h尿蛋白定量、SCr均高于阴性组(P＜0.05),ALB、BUN 水平低于阴性组(P＜0.05).PLA2R/THSD7A 阴性组治疗后 3、6、12个月的总有效率均高于阳性组(P＜0.05).PLA2R/THSD7A阳性组与阴性组不良反应比较无差异(P＞0.05).结论 肾病综合征患者PLA2R阳性率高,THSD7A阳性率低,且PLA2R和THSD7A表达情况与治疗效果相关.
Abstract
Objective To observe the expression of antiphospholipase A2 receptor(PLA2R)and type 1 platelet reactive protein 7A domain(THSD7A)in renal tissue of patients with nephrotic syndrome and analyze the relationship with the therapeutic effect.Methods Selected 98 patients with nephrotic syndrome admitted to the hospital from May 2020 to April 2022,detected the ex-pression of PLA2R and THSD7A in the renal tissue by immunuminescence method;combined glucocorticoid and immunosuppressive therapy for 2 months,according to the patients were di-vided into PLA2R negative and the positive group,the renal function group,and negative effects of PLA2R/THSD7A positive and negative group for 3,6 and 12 months.Results The positive expression of PLA2R was 61.22％(60/98)and negative expression was 38.78％(38/98);THSD7A positive expression was 33.67％(33/98)and negative expression was 66.33％(65/98).There was no difference in gender,age,duration,diabetes,hypertension and high fat a-mong the negative/positive groups(P＞0.05).The urinary protein quantification and SCr at 24 h were higher than the negative group(P＜0.05),and the ALB and BUN levels were lower than the negative group(P＜0.05).The total response rate in the PLA2R/THSD7A negative group was higher than that in the positive group(P＜0.05).No adverse effects between the PLA2R/THSD7A positive group and the negative group(P＞0.05).Conclusion PLA2R and THSD7A are low,and PLA2R and THSD7A expression are associated with treatment effect.
