摘要
目的 探讨产前母体血清甲胎蛋白(AFP)、游离β绒毛膜促性腺激素(Free-β-HCG)、雌三醇(uE3)检测对筛查胎儿染色体异常的临床价值.方法 选取我院2021年9月-2023年3月100例高风险孕妇进行无创DNA检测,根据染色体状态分为阳性组、阴性组.对比2组孕12周时血清AFP、Freeβ-HCG、uE3及NT值水平,Logistic回归分析血清各指标对胎儿染色体异常的影响因素;ROC分析孕12周时血清各指标联合检测对胎儿染色体异常的预测价值.结果 阳性组孕12周时血清AFP、uE3均低于阴性组、血清Freeβ-HCG及NT值均高于阴性组(P<0.05);分析认为孕12周时血清AFP、uE3为胎儿染色体异常的保护因素、Freeβ-HCG及NT值为胎儿染色体异常的危险因素(P<0.05);ROC分析孕12周时检测母体血清AFP、Freeβ-HCG、uE3水平对预测胎儿染色体异常的AUC分别为0.654、0.673、0.664,其联合检测对预测胎儿染色体异常的AUC为0.717.结论 产前母体血清AFP、Freeβ-HCG、uE3联合检测在胎儿染色体异常的筛查中均具有较高的预测价值,3者联合检测的预测效能更高,可广泛应用于早期临床筛查中.
Abstract
Objective To investigate the clinical value of maternal serum alpha-fetoprotein(AFP),free-beta-human chorionic gonadotropin(Free-β-HCG),and uniparental disomy(uE3)detection for screening fetal chromosomal abnormalities.Methods A total of 100 high-risk pregnant women in our hospital from September 2021 to March 2023 were selected for non-inva-sive DNA detection,and were divided into positive group and negative group according to chro-mosome status.The levels of serum AFP,Freeβ-HCG,uE3 and NT were compared between the two groups at 12 weeks of pregnancy,and the influencing factors of serum indexes on fetal chro-mosome abnormality were analyzed by Logistic regression.Diagnostic value of serum indexes combined with ROC analysis for fetal chromosomal abnormalities at 12 weeks of pregnancy.Re-sults At 12 weeks of pregnancy,serum AFP and uE3 in positive group were lower than those in negative group,serum Freeβ-HCG and NT values were higher than those in negative group(P<0.05).At 12 weeks of pregnancy,serum AFP and uE3 were protective factors of fetal chromo-some abnormality,and Freeβ-HCG and NT were risk factors of fetal chromosome abnormality(P<0.05).The AUCs of maternal serum AFP,Freeβ-HCG,and uE3 levels for predicting fetal chromosomal abnormalities at 12 weeks of gestation were 0.654,0.673,and 0.664,respective-ly,while the combined detection of these three markers had an AUC of 0.717 for predicting fetal chromosomal abnormalities.Conclusion The combined detection of maternal serum AFP,Freeβ-HCG,and uE3 in prenatal screening has high predictive value for fetal chromosomal abnor-malities,and the combined detection of the three has higher predictive efficiency and can be wide-ly used in early clinical screening.
