摘要

目的 探讨双重血浆分子吸附系统(DPMAS)联合血浆置换(PE)和131I治疗甲状腺功能亢进(甲亢)性重度高胆红素血症的临床效果.方法 报告1例甲亢性重度高胆红素血症病人,结合相关的文献复习,总结其临床特点及诊治经验.结果 本文病人在内科综合治疗的基础上,给予DPMAS联合PE和131I治疗,病人甲状腺功能及肝功能明显好转.结论 甲亢性重度高胆红素血症的诊断和治疗较为困难,DPMAS+PE人工肝治疗技术结合131I可能是在内科综合治疗基础上治疗甲亢性重度高胆红素血症的合理选择.

Abstract

Objective To investigate the clinical effectiveness of double plasma molecular adsorption system(DPMAS)com-bined with plasma exchange(PE)and 131I therapy in the treatment of hyperthyroidism with severe hyperbilirubinemia.Methods We reported a case of hyperthyroidism-induced severe hyperbilirubinemia,and reviewed related literature to summarize its clinical characteristics and diagnosis and treatment experience.Results After comprehensive routine treatment as well as DPMAS combined with PE and 131I therapy,the thyroid function and liver function of the patient were significantly improved.Conclusion Severe hyperbilirubinemia due to hyperthyroidism is difficult to diagnose and treat,and DPMAS+PE/artificial liver systems+131I treatment may be an appropriate complement to comprehensive routine treatment.
