摘要
目的 探讨曲安奈德联合595 nm脉冲染料激光(PDL)治疗美容缝合术后切口早期增生性瘢痕的效果.方法 选取2021年12月—2023年6月我院门诊收治的色素痣或体表肿物切除术后3个月内切口形成增生性瘢痕的病人56例.以曲安奈德联合595 nm PDL治疗者为观察组,595 nm PDL治疗者为对照组,每组28例病人.采用改良温哥华瘢痕量表、瘙痒程度数字评分法(NRS)和疼痛视觉模拟评分法(VAS)评价疗效.比较两组病人治疗3～6个月后疗效和不良反应发生率.结果 治疗后,观察组疗效和满意度均显著优于对照组(Z=2.6、3.5,P＜0.05),瘢痕色泽、柔软度、厚度以及VAS评分、NRS评分均显著低于对照组(t=3.41～12.70,P＜0.05).两组不良反应发生率差异无统计学意义(P＞0.05).结论 曲安奈德联合595 nm PDL治疗美容缝合术后早期切口增生性瘢痕临床疗效及满意度较好,可广泛应用于临床.
Abstract
Objective To investigate the efficacy of triamcinolone acetonide combined with 595 nm pulsed dye laser(PDL)in the treatment of hyperplastic scars in the early stage after cosmetic suture surgery.Methods This study was conducted a-mong 56 patients who underwent the resection of pigmented nevi or surface tumors at the outpatient service of our hospital from December 2021 to June 2023 and developed hyperplastic scars inincisions within 3 months after surgery.The patients treated with triamcinolone acetonide combined with 595 nm PDL were enrolled as observation group,and those treated with 595 nm PDL alone were enrolled as control group,with 28 patients in each group.The Modified Vancouver Scar Scale,Numerical Rating Scale(NRS)for pruritus,and Visual Analog Scale(VAS)for pain were used to evaluate the efficacy of treatment.The two groups were com-pared in terms of treatment outcome and the incidence rate of adverse events after 3-6 months of treatment.Results After treatment,compared with the control group,the observation group had significantly better treatment outcome and degree of satis-faction(Z=2.6,3.5;P＜0.05)and significantly lower scores of scar color/softness/thickness,VAS score,and NRS score(t=3.41-12.70,P＜0.05).There was no significant difference in the incidence rate of adverse reactions between the two groups(P＞0.05).Conclusion Triamcinolone acetonide combined with 595 nm PDL has good clinical efficacy and degree of satisfaction in the treatment of hyperplastic scars in the early stage after cosmetic suture surgery,and therefore,it holds promise for clinical ap-plication.
