良性阵发性位置性眩晕成功复位后残余症状的临床特征及风险因素
Clinical features of residual symptoms after successful repositioning maneuver of benign paroxysmal positional vertigo and related risk factors

摘要
目的 探讨良性阵发性位置性眩晕(BPPV)成功复位后残余症状的临床特征及风险因素.方法 回顾性分析2022年1-12月于我院神经内科就诊的109例成功复位的BPPV病人的临床资料,根据有无残余症状分组,比较两组临床资料的差异,应用二元Logistic回归分析方法筛选出BPPV成功复位后残余症状发生的风险因素.结果 109例BPPV病人成功复位后43例(39.45％)出现残余症状.单因素分析显示,外半规管嵴帽型、病程及复位次数是残余症状的潜在风险因素(x2=10.917～17.206,P＜0.01);二元Logistic回归分析显示,外半规管嵴帽型、病程及复位次数是残余症状的独立风险因素(OR=0.135～0.284,95％CI=(0.024～0.098)～(0.727～0.822),P＜0.05).结论 BPPV复位成功后残余症状的出现无性别差异,外半规管嵴帽型、长病程及多次复位者易出现残余症状.在临床中病人一旦确诊BPPV,应及时予以复位治疗.同时应提高每次手法复位成功率,减少复位次数.
Abstract
Objective To investigate the clinical features of residual symptoms after successful repositioning maneuver of be-nign paroxysmal positional vertigo(BPPV)and related risk factors.Methods A retrospective analysis was performed for the clinical data of 109 BPPV patients who received successful repositioning maneuver in Department of Neurology in our hospital from January to December 2022,and the patients were divided into two groups based on the presence of absence of residual symptoms.Clinical data were compared between the two groups,and the binary Logistic regression analysis was used to identify the risk fac-tors for the onset of residual symptoms after successful repositioning maneuver.Results Among the 109 patients with BPPV,43(39.45％)experienced residual symptoms after successful repositioning maneuver.The univariate analysis showed that lateral semicircular canal cupulolithiasis,duration of vertigo before treatment,and the number of repositioning maneuvers were potential risk factors(x2=10.917-17.206,P＜0.01),and the binary Logistic regression analysis showed that lateral semicircular canal cu-pulolithiasis,duration of vertigo before treatment,and the number of repositioning maneuvers were independent risk factors for re-sidual symptoms(OR=0.135-0.284,95％CI=(0.024-0.098)to(0.727-0.822),P＜0.05).Conclusion There is no sex difference in the onset of residual symptoms after successful repositioning maneuver of BPPV,and the patients with lateral semicir-cular canal cupulolithiasis,a long duration of vertigo before treatment,and multiple repositioning maneuvers are more likely to ex-perience residual symptoms.Once a patient is diagnosed with BPPV in clinical practice,he/she should be given timely repositioning maneuver,and the success rate of each repositioning maneuver should be increased to reduce the number of maneuvers.
