摘要

目的 探讨血浆前清蛋白(PAB)对老年新型冠状病毒肺炎(简称新冠肺炎)病人并发呼吸衰竭的影响.方法 采用横断面研究法,收集2022年12月-2023年1月入院的175例老年新冠肺炎病人的临床资料,根据入院时是否发生呼吸衰竭将病人分为呼吸衰竭组(呼衰组)和正常组,采用多因素Logistic回归分析法分析老年新冠肺炎病人并发呼吸衰竭的影响因素.结果 与正常组相比,呼衰组的舒张压、PAB、血红蛋白、体质量指数、清蛋白、预后营养指数、血小板计数和淋巴细胞计数均降低,差异具有统计学意义(t=2.09~3.05,Z=-3.24~-2.01,P<0.05);而呼衰组的女性病人比例、C反应蛋白和降钙素原显著增高,差异具有统计学意义(x2=2.65,Z=-2.77、-2.72,P<0.05).多因素 Logistic 回归分析显示,低 PAB(OR=0.892)、女性(OR=3.304)和高 C 反应蛋白(OR=1.011)是老年新冠肺炎病人并发呼吸衰竭的独立危险因素.结论 血浆PAB可作为老年新冠肺炎病人并发呼吸衰竭的潜在预测指标,有助于识别可能并发呼吸衰竭的老年新冠肺炎病人.

Abstract

Objective To investigate the effect of plasma prealbumin(PAB)on respiratory failure in elderly patients with coronavirus disease 2019(COVID-19).Methods In this cross-sectional study,clinical data were collected from 175 elderly pa-tients with COVID-19 who were admitted from December 2022 to January 2023,and according to the presence or absence of respi-ratory failure on admission,the patients were divided into respiratory failure group and normal group.The multivariate Logistic re-gression analysis was used to investigate the influencing factors for respiratory failure in elderly patients with COVID-19.Results Compared with the normal group,the respiratory failure group had significant reductions in diastolic blood pressure,PAB,hemo-globin,body massindex,albumin,prognostic nutrition index,platelet count,and lymphocyte count(t=2.09-3.05,Z=-3.24 to-2.01,P<0.05)and significant increases in the proportion of female patients and the levels of C-reactive protein(CRP)and pro-calcitonin(x2=2.65,Z=-2.77,-2.72,P<0.05).The multivariate Logistic regression analysis showed that low PAB(OR=0.892),female sex(OR=3.304),and high CRP(OR=1.011)were independent risk factors for respiratory failure in elderly pa-tients with COVID-19.Conclusion Plasma PAB can be used as a potential predictive factor for respiratory failure in elderly pa-tients with COVID-19 and may help to identify the elderly patients with COVID-19 who might experience the complication of re-spiratory failure.
