摘要
目的 探讨基于多学科协作诊疗(MDT)模式下TPS教学法在胃肠外科住院医师规范化培训中的应用效果及价值.方法 选取在湖州市中心医院胃肠外科轮转的住培医师共40名,随机分为观察组和对照组,每组20名.观察组采用MDT模式下的TPS教学法,对照组采用传统教学法,以消化道恶性肿瘤为教学内容,观察两组住培医师的学习成效,通过考核成绩和教学满意度调查问卷进行教学效果的分析比较.结果 观察组住培医师的单选题成绩、病例分析题成绩和总成绩均高于对照组,差异均有统计学意义(t分别=2.07、5.64、6.47,P均<0.05).观察组住培医师在学习兴趣、自学能力、合作能力、归纳和表达能力、临床诊疗信心、临床思维能力、综合能力和总体满意度方面的自我评价均高于对照组,差异均有统计学意义(t分别=3.19、4.18、5.16、4.49、2.44、8.10、8.49、3.01,P均<0.05).带教医生对观察组住培医师的分析辅助检查结果意义、制定综合治疗方案、前沿知识、主动性及合作性等方面的满意程度明显高于对照组,差异有统计学意义(t分别=3.89、1.99、2.96、2.12、3.19,P均<0.05),临床带教医师对基于MDT的TPS教学法总体满意度明显高于对照组,差异均有统计学意义(t=2.01,P<0.05).结论 在胃肠外科住院医师规范化培训中,MDT模式下TPS教学法效果优于传统教学模式,有助于住培医师在规范化培训过程中养成独立思考的习惯,拓展胃肠外科专业知识范围和提高临床实践综合能力.
Abstract
Objective Exploring the application effect and value of think-pair-share(TPS)teaching method based on multidisciplinary treatment(MDT)mode in standardized training of resident physicians in gastrointestinal surgery.Methods A total of 40 trained physicians rotating in the gastroenterology department of Huzhou central hospital were selected and randomly divided into an observation group and a control group,with 20 people in each group.The observa-tion group received the TPS teaching method based on MDT mode,while the control group received traditional teaching methods,taking the malignant tumors of the digestive tract as the teaching content.The learning effectiveness of the two groups of trained physicians was observed,and the teaching effectiveness was analyzed and compared through assessment scores and teaching satisfaction questionnaires.Results The single choice question scores,case analysis question scores,and total scores of the observation group were higher than those of the control group,and the differences were sta-tistically significant(t=2.07,5.64,6.47,P<0.05).The self-evaluation of resident training physicians in the observation group was higher than that in the control group in terms of learning interest,self-learning ability,cooperation ability,in-duction and expression ability,clinical diagnosis and treatment confidence,clinical thinking ability,comprehensive ability,and overall satisfaction,and the differences were statistically significant(t=3.19,4.18,5.16,4.49,2.44,8.10,8.49,3.01,P<0.05).The satisfaction of the observation group with the analysis of auxiliary examination results,the develop-ment of comprehensive treatment plans,cutting-edge knowledge,initiative,and cooperation was significantly higher than that of the control group,and the difference was statistically significant(t=3.89,1.99,2.96,2.12,3.19,P<0.05).The over-all satisfaction of clinical instructors with the TPS teaching method based on MDT was significantly higher than that of the control group(t=2.01,P<0.05).Conclusion In the standardized training of gastroenterology resident physicians,the TPS teaching method under the MDT mode is more effective than the traditional teaching mode,which helps the trained physicians develop the habit of independent thinking during the standardized training process,expand the scope of gastroenterology professional knowledge,and improve the comprehensive clinical practice ability.
