Study on viral load and biochemical indexes of HIV/HCV co-infected patients

目的:探讨HIV/HCV双感患者与HIV/AIDS患者病毒载量、生化指标之间的差异,以及与HIV单纯感染者HCV病毒载量(HCV-RNA)之间的关系,分析病毒载量与细胞免疫水平及生化指标间相关性,为HIV/HCV共感染的临床诊断、治疗提供依据.方法:收集伊宁市传染病医院2023年入院的HIV/AIDS感染者、HCV感染者、HIV/HCV双感患者血清样本,检测丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、总胆红素(TBIL)等生化指标;采用流式细胞术对CD4+T淋巴细胞进行计数,采用PCR技术检测HCV-RNA及HIV-RNA.结果:共收集HIV/AIDS感染者、HCV感染者和HIV/HCV共感染者及正常人群共481例,其中HIV/AIDS感染组192例,HCV感染组162例,HIV/HCV共感染组111例,正常对照组16例.HIV/HCV组的HIV-RNA检出率(51.35%)高于 HIV 组(25.52%)(P<0.001),HCV-RNA检出率高于 HCV 组(P<0.001),CD4+T淋巴细胞水平低于HIV/AIDS感染组(P<0.001),ALT和AST均极显著高于对照组、HIV组、HCV组(P<0.001),HIV病毒载量与CD4+T淋巴细胞计数及ALT、AST、GGT水平未存在相关性(P>0.05),与CD4+T%存在负相关性(P<0.05),HCV病毒载量与ALT、AST、GGT水平均存在正相关性(P<0.05),而HIV病毒载量与HCV病毒载量间未存在相关性(P=0.145).结论:HCV感染机体后,ALT、AST、GGT等肝功能指标异常增高,HIV/HCV共感染后机体免疫抑制加重,免疫功能更加低下,肝脏的损伤严重,机体对HIV及HCV病毒的复制控制能力下降.
Objective:To explore the differences in viral load and biochemical indicators between HIV/HCV patients with dual infection and HIV/AIDS patients,as well as the relationship between HCV viral load and HCV viral load(HCV-RNA)in HIV simplex infection patients,and analyze the correlation between viral load and cellular immunity level and biochemical indicators,so as to provide evidence for clinical diagnosis and treatment of HIV/HCV co-infection.Methods:Serum samples of HIV/AIDS infected persons,HCV infected persons and HIV/HCV co-infected persons admitted to Yining Infectious Disease Hospital in 2023 were collected to detect alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL)and other biochemical indicators.CD4+T lymphocytes were counted by flow cytometry and HCV-RNA and HIV-RNA were detected by PCR.Results:A total of 481 cases of HIV/AIDS infected,HCV infected,HIV/HCV co-infected and normal population were collected,including 192 cases of HIV/AIDS infected group,162 cases of HCV infected group,111 cases of HIV/HCV co-infected group and 16 cases of normal control group.HIV-RNA detection rate was higher in HIV/HCV group(51.35%)than in HIV group(25.52%)(P<0.001),the detection rate of HCV-RNA was higher than that of HCV group(P<0.001),CD4+T lymphocyte level was lower than HIV/AIDS-infected group(P<0.001),ALT and AST were significantly higher than those in control group,HIV group and HCV group(P<0.001),there was no correlation between HIV viral load and CD4+T lymphocyte count,ALT,AST,GGT levels(P<0.05)was negatively correlated with CD4+T%(P<0.05).HCV viral load was positively correlated with ALT,AST and GGT levels(P<0.05),and there was no significant correlation between HIV viral load and HCV viral load(P=0.145).Conclusion:ALT,AST,GGT and other liver function indicators are abnormally increased after HIV/HCV co-infection,the body's immune suppression is aggravated,the immune function is even lower,the liver damage is serious,and the body's ability to control the replication of HIV and HCV virus is weakened after HCV infection.
