摘要
目的:探讨血清降钙素原(PCT)、白细胞介素6(IL-6)、前白蛋白(PA)在新生儿细菌感染性疾病(NBI)中的表达意义及与T淋巴细胞亚群水平.方法:回顾性收集2022年1月-2023年1月我院收治的150例NBI新生儿作为研究对象并纳入NBI组,另选取150例在我院进行正常体检的健康新生儿作为对照组,对比两组新生儿血清PCT、IL-6、PA及T淋巴细胞亚群水平;Pearson相关系数分析PCT、IL-6、PA与T淋巴细胞亚群水平的相关性;绘制受试者操作特征曲线(ROC)分析PCT、IL-6、PA单一及联合检测在新生儿NBI中的检测意义.结果:相比于对照组,NBI组新生儿血清PCT、IL-6、CD3+、CD4+表达显著上升,而PA、CD8+水平显著下降(均P<0.05);Pearson相关系数分析显示PCT、IL-6均与CD3+、CD4+呈显著正相关关系,但与CD8+呈负相关(均P<0.05);而PA则与CD3+、CD4+呈显著负相关,但与CD8+呈正相关(均P<0.05);ROC分析显示PCT、IL-6、PA联合检测的曲线下面积(AUC)显著高于任意指标单独检测(均P<0.05),敏感度为86.00%,特异度为86.67%.结论:PCT、IL-6、PA在NBI新生儿中均具有特异性表达,且与新生儿T淋巴细胞亚群水平之间具有较强关联性,临床通过联合检测PCT、IL-6、PA对NBI的发生发展具有良好的诊断价值,值得临床推广使用.
Abstract
Objective:To investigate the significance of serum calcitoninogen(PCT),interleukin 6(IL-6),prealbumin(PA)expression and their relationship with T lymphocyte subpopulation levels in neonates with bacterial infectious disease(NBI).Methods:150 neonates with NBI admitted to our hospital from January 2022 to January 2023 were retrospectively collected,and 150 healthy neonates who underwent normal physical examination in our hospital were selected as the control group,and the levels of serum PCT,IL-6,PA and T lymphocyte subpopulation were compared between the two groups;Correlation between PCT,IL-6,PA and T-lymphocyte subpopulation levels was analyzed by Pearson correlation coefficient.;Receiver operating characteristic curve(ROC)curves was drawn to analyze the significance of single and combined PCT,IL-6,PA in neonatal NBI.Results:Compared with the control group,serum PCT,IL-6,CD3+,CD4+expression was significantly increased in neonates with NBI,while PA and CD8+levels were significantly decreased(both P<0.05);Pearson correlation coefficient analysis showed that PCT and IL-6 were both significantly positively correlated with CD3+and CD4+,but negatively correlated with CD8+(both P<0.05 The ROC analysis showed that the area under the curve(AUC)of the combined PCT,IL-6,and PA assay was significantly higher than that of any index alone(all P<0.05),with a sensitivity of 86.00%and specificity of 86.67%.Conclusion:PCT,IL-6,and PA are specifically expressed in neonates with NBI,and there is a strong correlation between them and the level of neonatal T-lymphocyte subpopulation.The clinical use of combined PCT,IL-6,and PA has good diagnostic value for the early assessment of the occurrence of NBI,and is worthy of clinical promotion.
