摘要

目的:探讨备孕妇女卵巢储备功能的评估预测中联合检测AMH、FSH和LH指标的临床价值.方法:于2021年3月-2022年3月阶段采集病例展开研究,对象为备孕妇女(育龄期女性)200例,根据年龄分为5组,分别是<25岁组(32例)、25~30岁组(58例)、31~35岁组(50例)、36~40岁组(41例)、41~45岁组(19例),均检测AMH、FSH和LH水平,比较各年龄阶段育龄期女性的AMH、FSH和LH水平差异;同时AMH水平≤1.0 ng/mL 的 132 例,以 AMH 水平分组为 0.6~1.0ng/mL、0.1~0.5 ng/mL、<0.1 ng/mL 共 3 组,另 AMH 水平>1.0 ng/mL的68例育龄期女性为对照组,对比各组的FSH和LH水平差异;根据受试者工作曲线(ROC)分析以上指标对卵巢储备功能的预测价值.结果:随着年龄增加AMH水平逐渐降低,与其他年龄阶段相比,>35岁的女性AMH水平下降程度更大(P<0.05),各年龄组的育龄期女性LH、FSH水平比较有统计学差异(P<0.05);低AMH水平 0.6~1.0 ng/mL、0.1~0.5 ng/mL、<0.1 ng/mL各组女性 LH、FSH 水平差异明显,<0.1 ng/mL 组 LH、FSH 水平明显高于0.6~1.0 ng/mL、0.1~0.5 ng/mL组(P<0.05),随着AMH水平的下降LH、FSH水平逐渐升高(P<0.05);ROC曲线分析,AMH、LH、FSH检测对卵巢储备功能预测均具有一定价值,AMH+LH+FSH检测灵敏度为0.880、特异度为 0.974,均高于单一 AMH 检测(0.805、0.083)、LH 检测(0.774、0.678)、FSH 检测(0.748、0.503).结论:备孕妇女联合检测AMH、FSH和LH指标可较好的评估育龄期女性卵巢储备功能,为备孕妇女合理安排孕育计划提供可靠参考.

Abstract

Objective:To explore the clinical value of combined detection of AMH,FSH and LH indicators in the evaluation and prediction of ovarian reserve function in women preparing for pregnancy.Methods:From March 2021 to March 2022,a study was conducted on 200 women of childbearing age who were divided into 5 groups based on age:<25 years old(32 cases),25~30 years old(58 cases),31~35 years old(50 cases),36~40 years old(41 cases),and 41~45 years old(19 cases).The levels of AMH,FSH,and LH were tested,and the differences in AMH,FSH,and LH levels among women of childbearing age at different age groups were compared.132 cases with AMH levels ≤1.0 ng/mL were further divided into three groups based on AMH levels:0.6~1.0 ng/mL,0.1~0.5 ng/mL,and<0.1 ng/mL.Additionally,68 women of childbearing age with AMH levels>1.0 ng/mL were used as the control group to compare the differences in FSH and LH levels between each group;Analysis of the predictive value of the above indicators to ovarian reserve function based on the receiver operating curve(ROC).Results:As age increased,AMH levels gradually decreased.Compared with younger age groups,women over 35 years old had a greater decrease in AMH levels(P<0.05).There were statistical differences in LH and FSH levels among women in each age group(P<0.05);There were significant differences in LH and FSH levels among females with AMH levels ≤ 1.0 ng/mL.The LH and FSH levels in the<0.1 ng/mL group were significantly higher than those in the 0.6~1.0 ng/mL,and those 0.1~0.5 ng/mL groups(P<0.05),and gradually increased with the decrease of AMH levels(P<0.05);ROC analysis shows that combined analysis of AMH,LH,and FSH have considerable value in predicting ovarian reserve function.The sensitivity and specificity of AMH+LH+FSH detection are 0.880 and 0.974 respectively,which are higher than single indicator detection(AMH 0.805,0.083;LH 0.774,0.678;FSH 0.748,0.503).Conclusion:The combined detection of AMH,FSH,and LH in women of childbearing age can effectively evaluate ovarian reserve function,providing reliable reference for females of a wide age range to organize their pregnancy plans reasonably.
