血清孕酮、ACTA、雌二醇、β-HCG联合检测对早期异位妊娠的预测价值

摘要
目的:研究早期异位妊娠诊断中联合检测血清孕酮(P)、血清激活素A(ACTA)、雌二醇(E2)、人绒毛膜促性腺激素(β-HCG)的价值.方法:筛选2018年12月-2022年12月早期异位妊娠57例为对象(观察组),57例宫内正常妊娠为对照组,检验相关指标,分析其差异.结果:对照组血清P、ACTA、E2、β-HCG比观察组比较更高(P＜0.05);联合诊断的阳性率(94.74％)比较单一指标的诊断阳性率61.40％、57.89％、56.14％、59.65％更高,联合诊断方式的准确度(94.74％)、灵敏度(94.74％)、特异度(97.92％)、阳性预测值(94.44％)、阴性预测值(90.48％)均高于单一检查指标.结论:血清孕酮、人绒毛膜促性腺激素、血清激活素A、雌二醇联合检测在诊断早期异位妊娠中准确度比较高,与正常子宫妊娠孕妇的差异明显,可以提升诊断的准确度,在临床中需要结合产妇的具体情况做出选择.
Abstract
Objective:To study the value of serum progesterone(P),human chorionic gonadotropin(β-HCG),serum activin A(ACTA)and estradiol(E2)in early ectopic pregnancy diagnosis.Methods:57 cases of early ectopic pregnancies from December 2018 to December 2022 were selected as the subjects(observation group),and 57 intrauterine normal pregnancies as the control group.Relevant indicators were tested and the differences were analyzed.Results:Serum P,ACTA,E2 and β-HCG in control group were higher than those in observation group(P＜0.05).The positive rate of combined diagnosis(94.74％)was higher than that of single index(61.40％,57.89％,56.14％,59.65％).The accuracy(94.74％),sensitivity(94.74％),specificity(97.92％),positive predictive value(94.44％)and negative predictive value(90.48％)of combined diagnostic method were higher than those of single examination index.Conclusion:The combined test of serum progesterone,human chorionic gonadotropin,serum activin A and estradiol is relatively accurate in the diagnosis of early ectopic pregnancy,with obvious differences from normal pregnancy.Therefore,combined test improves the accuracy of diagnosis with consideration of specific situation of pregnant women in clinical practice.
