摘要

目的:探讨流式细胞术(FCM)检测在急性白血病(AL)患者免疫表型表达的意义,分析FCM在AL患者中的临床应用价值.方法:回顾性分析2021年5月-2022年12月于江苏省人民医院血液病理诊断中心进行FCM检测的AL患者175例,进行免疫分型检测,分析不同AL患者的抗原表达情况.结果:AML患者114例,表达cMPO、CD34、CD117、HLA-DR、CD13、CD33、CD15、CD123、CD58、CD15、CD123、CD58、CD81、CD38、CD11c、CD71 等抗原;T-ALL 患者 10 例,表达 cCD3、CD34、CD10、CD81、CD38、CD71、CD2、CD5、CD7、CD4、CD1a、CD99、TDT 等抗原;B-ALL 51 例,表达 cCD79a、CD34、HLA-DR、CD13、CD33、CD19、CD58、CD10、CD20、CD22、CD81、CD66c、CD38、CD9、CD71 等抗原.AML 及 ALL 均存在跨系表达抗原,AML存在CD2、CD4、CD7、CD19、CD56等跨系抗原表达,ALL常见跨系抗原表达为HLA-DR、CD13、CD33、CD123、CD15等.通过流式细胞术还发现了罕见的血液系统恶性肿瘤母细胞性浆细胞样树突细胞肿瘤(BPDCN)3例.结论:FCM免疫分型能客观准确地检测出白血病细胞中抗原的表达情况,辅助判断白血病细胞类别及分化阶段,为白血病的治疗及预后提供了重要的实验室数据,具有重要的临床意义.

Abstract

Objective:To investigate the expression characteristics of flow cytometry immunophenotyping in patients with acute leukemia(AL),and to analyze the significance of flow cytometry in the patients of acute leukemia.Methods:The flow cytometry immunophenotyping results of 175 patients with initial AL admitted in Hematologic diagnostic center ofJiangsu province hospital from May 2021 to Dec 2022 were retrospectively analyzed,to analyze the expression of antigen in the patients of acute leukemia.Results:cMPO,CD34,CD117,HLA-DR,CD13,CD33,CD15,CD123,CD58,CD15,CD123,CD58,CD81,CD38,CD11c,CD71 were expressed in 114 patients of AML;cCD3,CD34,CD10,CD81,CD38,CD71,CD2,CD5,CD7,CD4,CD1a,CD99,TDT were expressed in 10 patients of T-ALL;cCD79a,CD34,HLA-DR,CD13,CD33,CD19,CD58,CD10,CD20,CD22,CD81,CD66c,CD38,CD9,CD71were expressed in 51 patients ofB-ALL,AML and ALL had cross-line expression antigens,cross-line antigens of AML expressed were CD2,CD4,CD7,CD19,CD56;Cross-line antigens ALL expressed were HLA-DR,CD13,CD33,CD123,CD15.Three cases of rare hematological malignancy(BPDCN)were identified by flow cytometry.Conclusion:The expression of antigens in leukemia cells can detect objectively and accurately with FCM immunotyping,FCM immunotyping also can assist to judging the type and differentiation stage of leukemia cells,providing important laboratory data for the treatment and prognosis of leukemia,which has important clinical significance.
