摘要

目的:探讨血清C反应蛋白(CRP)、纤维蛋白原(FIB)联合血小板分布宽度(PDW)对短暂性脑缺血发作后脑梗死的预测价值.方法:选取2021年1月-2022年12月永春县总医院收治的100例短暂性脑缺血发作后脑梗死患者作为研究对象,根据是否发生脑梗死将其分成发生组(n=36例)和未发生组(n=64例),收集患者的临床资料,检测两组受检者的血CRP、FIB以及PDW,采用多因素Logistic回归分析短暂性脑缺血发作后脑梗死的影响因素,采用受试者工作特征(ROC)曲线分析血CRP、FIB联合PDW对于在短暂性脑缺血发作后预测是否会发生脑梗死的价值.结果:①单因素分析结果显示:发作至入院时间、TG、血CRP、FIB和PDW等指标差异具有统计学意义(P<0.05).②多因素Logistic回归分析显示:短暂性脑缺血发作后脑梗死患者的危险因素有发作至入院时间、TG血CRP、FIB和PDW(P<0.05).③ROC曲线分析显示:血CRP、FIB联合PDW对短暂性脑缺血发作后脑梗死有一定的预测作用(血CRP:AUC=0.769,95%CI为0.673~0.865,其敏感度、特异度分别为0.750、0.734.FIB:AUC=0.789,95%CI 为 0.659~0.884,其敏感度、特异度分别为 0.528、0.937.PDW:AUC=0.716,95%CI 为0.617~0.814,其敏感度、特异度分别为0.833、0.578);且联合预测AUC(0.899)高于单个变量的AUC,提示联合预测短暂性脑缺血发作后脑梗死的价值和准确度更高.结论:血CRP、FIB联合PDW是短暂性脑缺血发作后脑梗死的危险因素,且血CRP、FIB联合PDW预测短暂性脑缺血发作后脑梗死的准确度高于单个变量,诊断价值更高.

Abstract

Objective:To explore the predictive value of blood CRP,FIB combined with PDW for cerebral infarction after transient ischemic attack.Methods:A total of 100 patients with post transient ischemic stroke(TIA)admitted to Yongchun County General Hospital from January 2021 to December 2022 were selected as the study subjects.They were divided into an occurrence group(n=36 cases)and a non occurrence group(n=64 cases)based on whether or not there was a stroke.Clinical data of the patients were collected,and the blood CRP,FIB,and PDW of the two groups of patients were measured.Multivariate logistic regression analysis was used to analyze the influencing factors of TIA post stroke stroke stroke stroke,The value of using receiver operating characteristic(ROC)curves to analyze blood CRP.FIB combined with PDW in predicting the occurrence of cerebral infarction after transient ischemic attacks.Results:The results of univariate analysis showed that there were statistically significant differences in indicators such as time from onset to admission,TG,blood CRP,FIB,and PDW(P<0.05)Multivariate logistic regression analysis showed that the risk factors for cerebral infarction in patients with transient ischemic attacks include time from onset to admission,TG blood CRP,FIB,and PDW(P<0.05)ROC curve analysis shows that the combination of blood CRP,FIB,and PDW has a certain predictive effect on cerebral infarction after transient ischemic attack(blood CRP:AUC=0.769,95%CI 0.673~0.865,sensitivity and specificity 0.750 and 0.734,respectively).FIB:AUC=0.789,95%CI 0.659~0.884,sensitivity and specificity 0.528 and 0.937,respectively.PDW:AUC=0.716,95%CI 0.617~0.814,sensitivity and specificity 0.833 and 0.578,respectively);Moreover,the joint prediction of AUC(0.899)is higher than the AUC of a single variable,indicating that the joint prediction of cerebral infarction after transient ischemic attack has higher value and accuracy.Conclusion:The combination of blood CRP,FIB,and PDW is a risk factor for cerebral infarction after transient ischemic attack,and the accuracy of predicting cerebral infarction after transient ischemic attack with blood CRP,FIB,and PDW is higher than that of a single variable,indicating higher diagnostic value.
