THE VALUE OF SERUM PROCALCITONIN AND ALBUM IN PREDICTING THE SEVERITY OF ACUTE PANCREATITIS
Objective To evaluate the clinical value of procalcitonin (PCT) and albumin (ALB) levels as early markers in severe acute pancreatitis (AP).Methods This study consisted of 215 patients with AP,among them,71 were with severe AP (SAP),72 were moderately SAP (MSAP) and 66 were with mild AP (MAP).Serum levels of PCT and ALB were measured within 24 hours of the onset.The association of PCT and ALB with APACHE Ⅱ score and CT severity grading was analyzed,Employing receiver operating characteristic (ROC) curve,the effectiveness of PCT and ALB in predicting the severity of AP was as sessed.Results Serum PCT level in SAP group was higher than that in MASP and MAP groups (F =4.869,P<0.01),The serum PCT level was positively correlated with APACHE Ⅱ scors (r =0.512,P < 0.01) and Balthazar CT grading (r =0.438,P <0.01).Serum ALB level in SAP group was lower than that in MSAP and MAP groups (F =5.231,P <0.01),and serum ALB level was negatively correlated with APACHEⅡ score (r=-0.441,P<0.01) and Balthazar CT grading (r=-0.497,P<0.01).The area under the ROC curve of PCT was 0.928,(preferably dividing point of 0.41 μg/L),the sensitivity and specificity in determination of SAP were 85.9% and 94.5 %,respectively;the.positive predictive value and negative predictive value were 93.8% and 87.3%,respectively.The area under the ROC curve of ALB was 0.928 (preferably dividing point of 33.45 mg/L),the sensitivity and specificity in determination of SAP were 83.1 % and 73.6 %,respectively.The sensitivity and specificity of PCT combined with ALB in predicting SAP were 80.2% and 76.1%,and positive and negative predictive values were 76.6% and 79.8%,respectively.Conclusion Early detection of serum procalcitonin and albumin in patients with acute pancreatitis can determine the severity of the disease and with or without a worsening trend of the disease,as soon as possible.
