3D TOF-MRA对颅内动脉开窗畸形诊断价值

目的 探讨颅内动脉开窗的三维时间飞越(3D TOF)-磁共振血管成像(MRA)影像学特征,提高对颅内动脉开窗的认识.方法 回顾性分析687例行3D TOF-MRA检查及血管后处理病人的影像学资料,观察颅内动脉开窗畸形的影像学特征.结果 本组687例中,共检出颅内动脉开窗畸形44例,检出率为6.40%.开窗畸形位置:椎动脉18例(左侧椎动脉10例,右侧8例),检出率为2.62%;基底动脉10例,检出率为1.46%;前交通动脉10例,检出率为1.46n0;大脑前动脉4例(左侧1例,右侧3例),检出率为0.58%;大脑中动脉2例(均为右侧),检出率为0.29%;未见颈内动脉及大脑后动脉开窗畸形.开窗畸形形态:裂隙型24例,凸透镜型19例,重复型1例.44例开窗畸形中,1例并发颅内动脉瘤,为梭形动脉瘤,位于椎动脉;1例并发短暂性脑缺血发作,开窗位于右侧大脑中动脉M1段.结论 3D TOF-MRA可以清晰显示颅内动脉开窗的位置、形态、类型、有无并发其他血管性疾病及与之的毗邻关系,有助于提高颅内动脉开窗畸形的检出率.
DIAGNOSTIC VALUE OF 3D TOF-MRA FOR IN INTRACRANIAL ARTERIAL FENESTRATION MALFORMATION
Objective To investigate 3D TOF-MRA imaging features of intracranial arterial fenestration (IAF) and improve the understanding of this kind of malformation.Methods Imaging data of 687 patients who underwent 3D TOF-MRA and vascular post treatments were reviewed retrospectively.The imaging features of IAF malformation were investigated.Results In this group of 687 cases,a total of 44 (6.40%) cases of IAF malformation were detected.The locations of IAF malformation were as follows:vertebral artery,18 cases (10 on the right side,eight on the left),detection rate 2.62%;basilar artery,10 cases,detection rate 1.46%;anterior communicating artery,10 cases,detection rate 1.46%;anterior cerebral artery,four cases (one on the left,three on the right),detection rate 0.58%;and middle cerebral artery,two cases (both on the right),detection rate 0.29%;no IAF malformations were found in internal carotid artery and posterior cerebral artery.Types of IAF were as follows:fissure type,24 cases;convex lens type,19 cases,and repeated type,one case.In 44 cases of IAF,one complicated with intracranial aneurysm (Fusiform aneurysm) located in vertebral artery,and one with transient ischemic attack of fenestration located in M1 segment of right middle cerebral artery.Conclusion 3D TOF-MRA can clearly show the position,shape and type of intracranial arterial fenestration,and with or without other vascular diseases and the adjacent relationship,which is helpful to raise the detection rate of intracranial artery fenestration malformation.
