纵隔肿瘤粒子治疗现状与展望

摘要
目的 综述纵隔肿瘤粒子治疗的原理、临床现状并展望其发展前景.方法 使用PubMed及中国知网数据库检索系统,以"纵隔肿瘤、放射治疗、质子治疗、碳离子放射治疗、硼中子俘获治疗、粒子治疗"等为中文关键词,"mediastinal tumors,proton therapy,carbon-ion radiotherapy,boron neutron capture therapy,particle therapy"为英文关键词进行检索.纳入标准:(1)纵隔肿瘤的粒子治疗;(2)纵隔淋巴瘤的粒子治疗;(3)胸腺瘤的粒子治疗.排除标准:(1)数据陈旧文献;(2)内容相似或重复.最终共纳入59篇文献.结果 纵隔肿瘤的多样性决定了其临床诊治的复杂性.质子、重离子、中子治疗均能够最大限度地保护危及器官,从而改善患者的生存状况,然而最佳的治疗方式仍存在争议.结论 粒子治疗在治疗纵隔肿瘤方面取得了迅速的发展,但仍需对患者进行筛选,并采取合理的措施推广粒子治疗.
Abstract
Objective To review the current principles and clinical status of mediastinal tumor particle therapy and look for-ward to its development.Methods The key words"mediastinal tumors,proton Therapy,carbon-ion radiotherapy,boron neutron capture therapy,particle therapy"were selected from PubMed and Chinese National Knowledge Infrastructure databases.Inclusion criteria were as follows:(1)Particle therapy for mediastinal tumors;(2)Particle therapy for medias-tinal lymphomas;(3)Particle therapy for thymoma.Exclusion criteria:(1)Relevant studies with outdated data;(2)Liter-ature with similar or duplicate content.A total of 59 articles were included.Result The diversity of mediastinal tumors determines the complexity of clinical diagnosis and treatment.Summarizing the application value of particle therapy in va-rious mediastinal tumors from the perspectives of proton,heavy ion,and neutron therapy,they can effectively safeguard vital organs and thereby improve patient survival,but the optimal treatment approach remains a subject of debate.Conclu-sions Particle therapy has experienced rapid development in the treatment of mediastinal tumors.However,patient se-lection and the implementation of reasonable measures for the widespread adoption of particle therapy are still necessary.
