HPV infection rate and subtype distribution among 326 824 cervical cancer screening and health check ups in Hunan Province from 2020 to 2022
Objective To analyze the infection status of human papillomavirus(HPV)in women in Hunan Province,as well as the relationship between HPV subtypes and pathological changes in cervical epithelial cells and tissues,to provide data support for local HPV epidemiology.Methods A total of 326 824 cervical epithelial cells from the population participating in cervical cancer screening and health examinations in Hunan Province from 2020 to 2022 were collected.Using fluores-cence PCR technology to detect their HPV typing,and statistically analyze the HPV infection status of different age groups and regions;Among them,9 940 cases underwent thin-prep cytology test or pathological examination to analyze the relationship between their HPV infection status and positive results in cell pathology.Results The overall infection rate of HPV was 10.9%(35 646/326 824),with monotypic infection accounting for 81.9%(29 201/35 646)and multi se-vere infection accounting for 18.1%(6 445/35 646);The top 6 types with a single type proportion were HPV52(20.5%),HPV58(16.0%),HPV16(12.9%),HPV53(11.9%),HPV81(8.7%),and HPV70(8.6%).The infection rate in the ≥60 years old group was the highest[15.7%(3672/23 315)].Among the HPV positive population,there was a statistically significant difference in the proportion of multiple severe infections among different age groups,x2=365.910,P<0.001.According to the source of cases,the highest infection rate was 13.1%(2 089/15 982)in Zhangjiajie City,and the lowest was 8.7%(3 219/37 058)in Yongzhou City.The positive rate of tissue and cytology in single type infections was 47.1%(3 618/7 683),while the positive rate in multiple severe infections was 58.8%(1 326/2 257),x2=94.867,P<0.001.Among 7 683 cases of single type HPV infection,the positive rates of HPV39,HPV66,HPV16,HPV6,HPV33,HPV42,HPV59 and HPV58 in tissue and cytology were exceeded 50%.Conclusions HPV infection rate among people over 60 years old is the highest,and efforts should be strengthened to prevent and control HPV infec-tion in this population.HPV52 and HPV58 are the most common genotypes in the entire population.Multiple severe HPV infections are more likely to lead to the occurrence of cervical diseases.
