Analysis of the disease burden of cancer in children aged 0-14 years in China from 1990 to 2019 and its changing trend
Objective To investigate the changing trend and main sources of pediatric cancer disease burden in 0-14 year old children in China from 1990 to 2019.Methods Data from Global Health Data Exchange(GHDx)database were used to analyze the incidence,mortality and disability-adjusted life year(DALY)rates of pediatric cancer in China and the world from 1990 to 2019 by sex and age.The Joinpoint method was used to analyze the annual percentage change(APC)and av-erage annual percentage change(AAPC)in pediatric cancer incidence,mortality and DALY rates.Results From 1990 to 2019,all indicators of the burden of childhood tumor disease in China showed a downward trend,with the average annual change of incidence rate of-3.28%(-3.61%,-2.95%),the average annual change of mortality rate of-3.29%(-3.53%,-3.05%),and the average annual change of DALY rate of-3.29%(-3.52%,-3.05%),all P<0.001;The overall trend of each indicator was divided into 6 sections.The incidence rate decreased by 6.58%,the years of lived with disability(YLD)rate decreased by 22.89%,which were lower than the global overall decrease rate,the mortality rate decreased by 62.17%,the DALY rate decreased by 62.04%,and the years of life lost(YLL)rate decreased by 60.7%,which were higher than the global overall decrease rate.The incidence in China was at a low level,but the mortality and DALY rates were much higher than those in developed countries such as the United States.Among all age groups,the indexes of disease burden in 0-28 d group were the highest,and the indexes showed a decreasing trend with the increase of age.There were gender differences in disease burden indicators.In China,leukemia was the highest in in-cidence,mortality,and DALY rates of childhood tumors,and the order of incidence rates in childhood cancer exhibited a gender difference.Conclusions The burden of cancer in children in China showed a decreasing trend from 1990 to 2019.Premature death was the main source of the burden of cancer,and there were gender and age differences in the burden.
