纳米药物递送系统在胃癌治疗中的研究进展

目的 综述纳米药物递送系统在胃癌治疗中的研究现状,阐述其在胃癌治疗中的作用机制,为纳米药物递送系统在胃癌治疗中的应用提供参考.方法 以"Nanomedicine Delivery System,Gastric cancer,Treatment"为英文关键词,"纳米药物递送系统、胃癌、治疗"为中文关键词,检索PubMed和中国知网数据库2009-12-01-2023-12-01相关文献.纳入标准:(1)相关动物实验;(2)相关临床试验;(3)纳米药物递送系统在胃癌治疗中的作用机制研究;(4)使用于胃癌治疗中的纳米药物递送系统载体;(5)纳米药物递送系统在胃癌治疗中的作用价值和应用现状;(6)纳米药物递送系统在胃癌治疗中的临床转化.排除标准:(1)内容相似或重复的文献;(2)研究机制模糊及存在争议的文献.最终共纳入文献74篇.结果 基于纳米药物递送系统的胃癌治疗作用机制主要是提高药物靶向性,延缓药物在体内的循环时间,减少药物在胃酸环境中的降解,调节胃癌细胞中的活性氧水平,增强机体抗肿瘤的免疫应答.基于此,纳米药物递送系统可实现多药递送,延缓耐药性,促进联合应用,减少毒副作用,增强术前和术中影像指导,从而增强疗效,为目前胃癌药物治疗方法存在的单药疗效差、毒副作用多和易产生耐药性等弊端提供一种极具潜力的解决策略.结论 随着新型纳米药物递送系统的研发和多项基于纳米药物递送系统治疗的动物实验和临床试验的开展,纳米药物递送系统在胃癌治疗中已初步取得一定成效.
Research progress of nanomedicine delivery system in the treatment of gastric cancer
Objective To review the research status of nanomedical drug delivery system in the treatment of gastric cancer,and explain its mechanism of action in the treatment of gastric cancer,which can provide reference for the future research prospect and clinical application potential of nanomedical drug delivery system in the treatment of gastric cancer.Methods With"Nanomedicine Delivery System,Gastric cancer,Treatment"as the keywords,the relevant literature of PubMed and CNKI were retrieved from December 2009 to December 2023.Inclusion criteria were as follows:(1)relevant animal experiments;(2)relevant clinical trials;(3)study on the mechanism of nano drug delivery system in the treatment of gastric cancer;(4)nano-drug delivery system carriers used in the treatment of gastric cancer;(5)the value and appli-cation status of nano-drug delivery system in the treatment of gastric cancer;(6)clinical transformation of nano-drug de-livery system in the treatment of gastric cancer.Exclusion criteria were:(1)articles with similar or repeated content;(2)the research mechanism is ambiguous and controversial.A total of 74 literatures were included.Results The thera-peutic mechanism of gastric cancer based on nano drug delivery system is mainly to improve drug targeting,delay drug circulation time in the body,reduce drug degradation in gastric acid environment,regulate the level of reactive oxygen species in gastric cancer cells,and enhance the anti-tumor immune response.Based on this,the nano-drug delivery system can realize multi-drug delivery,delay drug resistance,promote combined application,reduce side effects,enhance preop-erative and intraoperative image guidance,and thus enhance efficacy,providing a promising solution strategy for the shortcomings of current drug therapy for gastric cancer such as poor efficacy of single drug,many side effects,and easy drug resistance.Conclusion With the development of novel nanomedical drug delivery system and the development of a number of animal and clinical trials based on nanomedical drug delivery system therapy,nanomedical drug delivery system has emerged in the treatment of gastric cancer.
