64排螺旋CT对Lemmel’s综合征的诊断价值

摘要
目的：探讨64排螺旋 CT 对 lemmel 综合征的诊断价值。方法对36例诊断明确的 lemmel综合征患者 CT 资料结合病史、实验室检查及术后病理进行回顾性分析。结果36例 lemmel 患者中发现十二指肠乳头旁憩室36个,直径在9~38 mm 之间,其中30例为薄壁憩室,内可见炎症渗出,6例憩室壁增厚；十二指肠乳头旁憩室表现为十二指肠环内侧囊袋状突出于十二指肠小弯侧壁,其内多为气体密度影,增强扫描显示憩室壁与十二指肠壁强化一致,腔内无强化。①所有患者胆总管表现为不同程度的扩张,胆总管直径在11~20 mm 之间；②10例患者表现为胰管扩张,其中6例患者合并胰腺肿胀及胰腺周围渗出,周围脂肪间隙模糊；③6例患者表现出胆总管及肝管结石,结石直径为2~20 mm。结论64排螺旋 CT 对lemmel 综合征的诊断及指导临床治疗具有重要意义。
Abstract
Objective To explore the diagnostic and treatment value of MSCT on Lemmel's syndrome. Methods All 36 patients with Lemmel's syndrome by pathologically and clinical proven were underwent non -contrast and enhanced CT scan. CT features were analyzed combined with the medical history. Results There were 36 periampullarys diverticula in 36 patients, the diameter of the diverticula ranged from 9 to 38mm. The thin wall of diverticula was in 30 cases, with inn wall inflammatory exudation. The thick wall was in 6 cases. All PAD were located in the inside duodenum, the diverticula toward to sidewall of descending duodenum, CT scan showed the gas density and gas-fluid level. The wall equivalent enhancement between diverticula and duodenum, but the cavities had no enhancement. (1) All patients' common bile duct was different enlarged. the diameter of CBD ranges from 11~ 20mm; (2) Pancreatic main duct was enlarged in 10cases. (3) 6 cases CT showed pancreas swelling and around effusion. (4) 6 cases CT showed choledocholithiasis, the diameter of stone ranged from 2~20mm. Conclusions MSCT can not only make an accuracy diagnosis combined with the medical history on Lemmel's syndrome, but also can make a important help for treatment.
