摘要
目的 系统评价肌内效贴对全膝关节置换术(TKA)术后患者康复疗效的影响.方法 系统检索PubMed、The Cochrane Library、Web of Science、Ovid、中国生物医学文献服务系统(SinoMed)、中国知网、万方、维普数据库中关于肌内效贴对TKA术后患者康复疗效的随机对照试验,检索时间为建库至2023 年6 月,由2 名研究者独立对纳入文献进行质量评价及数据提取与汇总,采用Review Manager 5.3 软件进行Meta分析.结果 最终纳入文献8 篇,涉及患者538 例,其中肌内效贴组264 例,对照组274 例.Meta分析结果显示:肌内效贴组患者与对照组患者相比,在改善患肢膝关节活动角度ROM[MD =5.21,95%CI(1.31,9.11),P =0.009]、减轻术后患肢肿胀程度[MD =-3.04,95%CI(-5.28,-0.80),P =0.008]、减轻疼痛VAS评分[MD =-0.64,95%CI(-1.06,-0.22),P =0.003]、改善膝关节HSS功能评分[MD = 4.63,95%CI(1.14,8.12),P =0.009]方面差异有统计学意义,在起立-行走测试TUGT[MD =-2.19,95%CI(-5.26,0.89),P =0.16]方面差异无统计学意义.结论 TKA术后患者使用肌内效贴可提升术后患肢膝关节功能状态,降低疼痛体验及肿胀程度,表明肌内效贴可能是一种更具吸引力的术后康复干预方式.建议可综合考虑患者个体差异、实际临床场景与患者意愿等多因素应用肌内效贴,提升患者术后康复疗效,实现患者受益最大化.
Abstract
Objective To systematically evaluate the effects of kinesio taping on the rehabilitation efficacy of patients af-ter total knee arthroplasty(TKA).Methods A systematic search of randomized controlled trials on the efficacy of kinesio taping on rehabilitation of patients after TKA in PubMed,The Cochrane Library,Web of Science,Ovid,SinoMed,CNKI,Wanfang,and VIP databases was conducted from the establishment of the library to June 2023.Quality assessment,data extraction and summarization of the included literature were independently performed by two investigators.Meta-analysis was performed using Review Manager 5.3 software.Results 8 articles were finally found with 538 patients,including 264 in the experimental group and 274 in the con-trol group.Meta-analysis showed that patients in the kinesio taping group,when compared with patients in the control group,had a greater effect on improving the ROM of the knee flexion angle of the affected limb[MD = 5.21,95%CI(1.31,9.11),P = 0.009],decreasing the degree of swelling of the affected limb in the postoperative period[MD =-3.04,95%CI(-5.28,-0.80),P =0.008],reduction in pain VAS scores[MD =-0.64,95%CI(-1.06,-0.22),P =0.003],and im-provement in knee HSS function scores[MD =4.63,95%CI(1.14,8.12),P =0.009].There were statistically different,and no statistically significant difference was found in the get-up-and-go test TUGT[MD =-2.19,95%CI(-5.26,0.89),P =0.16].Conclusion Combined with current evidence that the use of kinesio taping in patients after TKA improves the functional status of the knee joint of the affected limb and reduces pain experience and swelling,suggesting that intramuscular-effective patches may be a more appealing postoperative rehabilitation intervention.It is suggested that the use of kinesio taping can be applied by taking into account individual patient differences,actual clinical scenarios,and patients'wishes to improve the efficacy of postoperative rehabil-itation and maximize the benefits to patients.
