血清CF-6、HSP-70表达对慢性心力衰竭患者病情及预后的评估价值

摘要
目的 探究血清线粒体偶联因子-6(CF-6)、热休克蛋白70(HSP-70)表达对慢性心力衰竭(CHF)患者病情及预后的评估价值.方法 选取2020 年12 月至2021 年12 月我院收治的CHF患者106 例(CHF组),根据美国纽约心脏病协会(NYHA)心功能分级标准分为:NYHA Ⅱ级 36 例(Ⅱ级组),NYHA Ⅲ级 38 例(Ⅲ级组),NYHA Ⅳ级 32 例(Ⅳ级组);根据预后情况分为预后良好组54 例,预后不良组52 例;选择同期我院体检健康的 106 例志愿者为对照组.采用酶联免疫吸附测定(ELISA)法检测血清中CF-6、HSP-70 的表达水平;多因素Logistic回归分析影响CHF预后的危险因素;受试者工作特征(ROC)曲线分析血清中CF-6、HSP-70 对CHF预后的评估价值.结果 与对照组相比,CHF组血清中CF-6、HSP-70 表达水平显著升高(P<0.05).CHF患者Ⅳ级组血清CF-6、HSP-70 表达水平显著高于Ⅱ级组和Ⅲ级组,Ⅲ级组表达水平显著高于Ⅱ级组(P<0.05).与预后良好组相比,预后不良组CF-6、HSP-70 水平显著升高(P<0.05).预后不良组血肌酐水平显著高于预后良好组(P<0.05).多因素Logistic回归分析显示,CF-6、HSP-70、血肌酐表达水平是影响CHF预后的危险因素;ROC分析显示,血清CF-6、HSP-70 水平联合预测CHF预后的AUC高于CF-6、HSP-70 单独预测的AUC值(P<0.05).结论 CHF患者血清CF-6、HSP-70 表达水平显著升高,并随着病情加重而升高,二者对CHF患者的预后评估有一定的临床价值.
Abstract
Objective To investigate the value of serum mitochondrial coupling factor 6(CF-6)and heat shock protein 70(HSP-70)expression in evaluating the condition and prognosis of patients with chronic heart failure(CHF).Methods A total of 106 CHF patients admitted to our hospital from December 2020 to December 2021 were regarded as the CHF group,according to the New York Heart Association(NYHA)cardiac function grading standards,36 cases of NYHA grade Ⅱ were classified as the grade Ⅱ group,38 cases of NYHA gradeⅢ were classified as the gradeⅢ group,and 32 cases of NYHA gradeⅣ were classi-fied as the gradeⅣgroup;according to the prognosis,54 cases were included into a good prognosis group and 52 cases into a poor prognosis group;106 healthy volunteers who underwent physical examination during the same period in our hospital were regarded as the control group.Enzyme linked immunosorbent assay(ELISA)was applied to detect the expression levels of CF-6 and HSP-70 in serum;multivariate Logistic regression was applied to analyze the risk factors affecting the prognosis of CHF;and receiver oper-ating characteristic(ROC)curve was applied to analyze the prognostic value of CF-6 and HSP-70 in serum for CHF.Results The expression levels of CF-6 and HSP-70 in the serum of the CHF group were obviously increased compared to the control group(P<0.05).The expression levels of CF-6 and HSP-70 in serum of patients with CHF in grade Ⅳ group were obviously higher than those in grade Ⅱ and Ⅲ groups,and the expression levels in grade Ⅲ group were obviously higher than those in gradeⅡ group(P<0.05).Compared with the good prognosis group,the levels of CF-6 and HSP-70 in the poor prognosis group were obviously increased(P<0.05).Serum creatinine level in poor prognosis group was significantly higher than that in good prognosis group(P<0.05).Multivariate Logistic regression analysis showed that CF-6,HSP-70 and serum creatinine expression were risk factors affecting the prognosis of CHF.ROC analysis showed that the AUC value predicted by serum CF-6 combined with HSP-70 for the prognosis of CHF was higher than that predicted by CF-6 and HSP-70 alone(P<0.05).Conclusion The expression levels of CF-6 and HSP-70 in the serum of CHF patients are obviously increased,and they increase with the aggravation of the disease.The two have certain clinical value in evaluating the prognosis of CHF patients.
