Objective To analyze the predictive value of serum uric acid(SUA)and β2 microglobulin(β2MG)levels in patients with sepsis complicated with acute kidney injury(AKI)and poor prognosis.Methods A retrospective study was con-ducted to select the clinical data of 120 patients with sepsis admitted to our hospital from June 2021 to June 2023 as the study sub-jects.Patients were grouped according to the occurrence and prognosis of AKI.All patients were tested for serum SUA and β2MG levels on admission.The baseline data of the two groups were statistically analyzed and compared.The baseline data of the two groups were statistically analyzed and compared.Binary Logistic and Cox regression were used to analyze the relationship between serum SUA and β2MG levels and AKI and poor prognosis in patients with sepsis.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum SUA and β2MG levels for AKI and poor prognosis in patients with sepsis.The dose-response relationship between serum SUA,β2MG levels and poor prognosis in patients with sepsis was analyzed by restricted cubic spline method combined with spline function and Logistic regression.Results Among 120 patients with sepsis,a total of 52 cases were studied.Among those,34 cases were diagnosed by Scr standard,18 cases were diagnosed by urine volume.The AKI grading of patients was 19 cases in stage 1,28 cases in stage 2 and 5 cases in stage 3.The results of binary Logistic regression anal-ysis showed that malnutrition,high expression of APACHE Ⅱ,SOFA,SUA and β2MG were risk factors for AKI in patients with sep-sis(OR>1,P<0.05);High expression of serum albumin is a protective factor for AKI in patients with sepsis(OR<1,P<0.05);Cox regression analysis showed that malnutrition,high expression of APACHE Ⅱ,SOFA,SUA and β2MG were risk factors for poor prognosis in patients with sepsis(HR>1,P<0.05);High expression of serum albumin was a protective factor for poor prognosis in patients with sepsis(HR<1,P<0.05).The results of ROC curve showed that the AUC of serum SUA,β2MG levels and combined detection in evaluating sepsis patients with AKI and poor prognosis were all>0.70,which had certain evaluation val-ue,and the combined detection had the highest value.There was a linear dose-response relationship between serum SUA,β2MG and poor prognosis in patients with sepsis(P<0.05).Serum SUA and β2 MG were positively correlated with poor prognosis in pa-tients with sepsis.Especially when SUA>432.96 μmol/L and β2MG>5.10 mg/L,the risk of poor prognosis in patients with sep-sis increased with the increase of serum SUA and β2 MG levels.Conclusion Serum SUA and β2MG levels are closely related to AKI and poor prognosis in patients with sepsis.The levels of both can effectively predict the occurrence of AKI and poor prognosis in patients with sepsis.Clinically,by detecting the levels of both,targeted interventions can be taken for patients with high risk of AKI and poor prognosis to improve the prognosis of patients.
