摘要

目的 探究甲强龙治疗免疫球蛋白A(IgA)肾病模型大鼠后,对其骨质疏松症发生的影响及可能作用机制.方法 36只SD大鼠随机分为对照组、模型组(建立IgA肾病模型,尾静脉注射生理盐水)、给药组(建立IgA肾病模型,尾静脉注射30 mg/kg甲强龙),每组12只.收集各组大鼠24 h尿液,采用全自动尿液分析仪检测尿蛋白;收集腹动脉血并分离血清,全自动生化分析仪检测各组大鼠血清中肌酐(CRE)、尿素氮(BUN)水平,ELISA法检测各组大鼠血清中抗酒石酸酸性磷酸酶-5b(TRACP-5b)表达;HE染色观察各组大鼠肾脏组织与股骨组织病理学变化;免疫组化染色检测股骨组织TRACP表达;Western Blot检测各组大鼠股骨组织中Toll样受体4(TLR4)/核因子κB(NF-κB)通路相关蛋白表达.结果 与对照组比较,模型组大鼠尿蛋白显著升高(P<0.05),血清中CRE、BUN显著升高(P<0.05),肾组织内肾小球、肾小管发生明显损伤,而血清中TRACP-5b含量未发生显著变化(P>0.05),股骨组织内骨小梁结构完整,TRACP表达水平以及TLR4、NF-κB、p-NF-κB蛋白相对表达水平均未发生显著变化(P>0.05);与模型组比较,给药组大鼠尿蛋白及血清中CRE、BUN显著降低(P<0.05),肾小球和肾小管病变明显减轻、间质炎细胞浸润数量减少,但血清中TRACP-5b含量显著升高(P<0.05),股骨组织中骨小梁结构明显稀疏变细且成骨细胞减少,TRACP表达水平显著增高(P<0.05),TLR4与p-NF-κB蛋白相对表达水平显著上调(P<0.05).结论 甲强龙能够改善IgA肾病模型大鼠肾脏损伤,但会促进TRACP表达致使骨质疏松症发生,该作用可能与激活TLR4/NF-κB通有关.

Abstract

Objective To explore the effect of methylprednone on the occurrence of osteoporosis in immunoglobulin A(IgA)nephropathy model rats and the possible mechanism.Methods 36 SD rats were randomly divided into control group,model group(IgA nephropathy model was established and normal saline was injected into tail vein),administration group(IgA nephropa-thy model was established and 30 mg/kg methylprednone was injected into tail vein),with 12 rats in each group.The urine of each group was collected for 24 h,and the urine protein was detected by automatic urine analyzer.Abdominal arterial blood was collected and serum was separated.The serum creatinine(CRE)and urea nitrogen(BUN)levels were detected by automatic biochemical analyzer,and the serum tartrate-resistant acid phosphatase-5b(TRACP-5b)content was detected by ELISA.The histopathological changes of kidney and femur were observed by HE staining.The expression of TRACP in femoral tissue was detected by immuno-histochemical staining.The expression of Toll-like receptor 4(TLR4)/nuclear factor κB(NF-κB)pathway-related proteins in fe-mur tissues of rats in each group was detected by Western Blot.Results Compared with the control group,urine protein of model group was significantly increased(P<0.05),the levels of CRE and BUN in serum were significantly increased(P<0.05),glo-meruli and renal tubules were significantly injured in renal tissue,while serum TRACP-5b content was not significantly changed(P>0.05),and the trabecular structure of femur tissue was intact,TRACP expression level and relative expression levels of TLR4,NF-κB and p-NF-κB protein were not significantly changed(P>0.05).Compared with the model group,urine protein and CRE and BUN in serum in the administration group were significantly decreased(P<0.05),the lesions of glomeruli and renal tu-bules were significantly reduced,the number of interstitial inflammation cells was decreased,but the content of TRACP-5b in the serum was significantly increased(P<0.05),the bone trabecular structure in the femur tissue was sparse and thin and osteoblasts were decreased,TRACP expression level was significantly increased(P<0.05),and the relative expression level of TLR4 and p-NF-κB protein was significantly up-regulated(P<0.05).Conclusion Methylprednone can improve kidney injury in IgA ne-phropathy model rats,but promote the expression of TRACP leading to osteoporosis,which may be related to the activation of TLR4/NF-κB.
