Objective To compare hemostatic efficacy of tranexamic acid(TXA)and gelatin in the perioperative man-agement of bone and soft tissue tumor surgery, aiming to provide a more economical and more effective hemostatic option for pa-tients.Methods A controlled trial was conducted, enrolling 76 patients undergoing bone and soft tissue tumor surgery between Oc-tober 2019 and January 2022.Based on intraoperative hemostatic interventions, patients were divided into TXA group(n=50,re-ceived intravenous infusion of 1 g tranexamic acid 30 minutes preoperatively)and gelatin group(n=26,local application of ap-proximately 2 ml gelatin at bleeding sites during surgery).Primary outcomes included hemoglobin, hematocrit, and platelet count.Secondary outcomes encompassed inflammatory markers, coagulation profiles, fibrinolytic markers, estimated intraoperative blood loss, and hospitalization duration. Results On postoperative day 1,hemoglobin were(118.94±16.79)g/L in TXA group and(116.46±16.81)g/L in gelatin group, with no statistically significant difference(P=0.984).The cost of hemostatic drugs was lower in the tranexamic acid group than in the gelatin group(P<0.05).No significant differences were observed in other primary or secondary outcomes(all P>0.05).Conclusion Tranexamic acid and gelatin demonstrated comparable hemostatic efficacy in the perioperative period of bone and soft tissue tumor surgery. However, considering its lower cost, tranexamic acid may represent a more cost-effective hemostatic option for patients undergoing bone tumor surgery.
