低分子肝素钙联合双嘧达莫对下肢骨折术后下肢深静脉血栓的疗效分析

摘要
目的:探讨在下肢骨折术后下肢深静脉血栓治疗中采用低分子肝素钙联合双嘧达莫的临床价值.方法:选取本院2020年 1 月—2021 年 3 月收治的 62例下肢骨折术后下肢深静脉血栓患者为研究对象,采取随机数字表法将其分为对照组与观察组,各31例.对照组采用低分子肝素钙皮下注射治疗,观察组在对照组基础上联合双嘧达莫治疗.比较 2 组患者的临床疗效、血液流变学指标、血清炎性因子水平、疼痛视觉模拟(VAS)评分及不良反应发生情况.结果:观察组治疗总有效率为93.55%,高于对照组的 74.19%,差异有统计学意义(P＜0.05).治疗 14d后,观察组各项血液流变学指标水平均低于对照组,组间差异有统计学意义(P＜0.05).治疗 7、14d后,观察组VAS评分均低于对照组,组间差异有统计学意义(P＜0.05).治疗 14d后,观察组血清白细胞介素-1、白细胞介素-6、白细胞介素-8、肿瘤坏死因子-α水平均低于对照组,组间差异有统计学意义(P＜0.05).2 组不良反应发生率比较,差异无统计学意义(P＞0.05).结论:在下肢骨折术后下肢深静脉血栓治疗中,采用低分子肝素钙联合双嘧达莫效果确切,能够有助于改善患者血液指标水平,缓解术后的炎症反应,促进术后康复.
Abstract
Objective:To investigate the clinical value of low molecular weight heparin calcium combined with dipyridamole in the treatment of deep venous thrombosis of lower extremity after lower extremity fracture.Methods:62 patients with deep venous thrombosis of lower extremity after lower limb fracture treated in our hospital from January 2020 to March 2021 were selected as the study objects,and were divided into a control group and an observation group by random number table method,with 31 cases in each group.The control group was treated with subcutaneous injection of low molecular weight heparin calcium,and the observation group was treated with dipyridamole on the basis of control group.The clinical efficacy,hemorheology index,serum inflammatory factor level,pain Visual Analogue Scale(VAS)score and the occurrence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 93.55%,which was higher than 74.19% of the control group,the difference was statistically significant(P<0.05).14 d after treatment,the levels of hemorheology indexes in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).7 and 14 d after dtreatment,VAS scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).14 d after treatment,serum levels of interleukin-1,interleukin-6,interleukin-8 and tumor necrosis factor-α of the observation group were lower than those of the control group,with statistical significance between the two groups(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:In the treatment of lower extremity deep vein thrombosis after lower extremity fracture,low molecular weight heparin calcium combined with dipyridamole has a definite effect,which can help to improve the blood index level of patients,alleviate postoperative inflammation and promote postoperative rehabilitation.
