摘要
目的:分析持续封闭负压引流技术(VSD)联合自体富血小板血浆凝胶(APG)在糖尿病慢性难愈合创面植皮治疗中的应用.方法:选取内蒙古自治区第三医院 2021年2 月—2022 年 6 月收治的 80例糖尿病慢性难愈合创面患者为研究对象.随机分为观察组(n=37)和对照组(n=43).对照组清创术后采用VSD覆盖创面,观察组清创后将APG均匀的敷在创面上,再用VSD覆盖创面,待 2 组创面达到移植皮片的条件时进行植皮术.比较 2 组创面植皮前细菌清除度、植皮前肉芽组织增长率、植皮前肉芽组织生长厚度、创面愈合率、创面愈合速率和疗效指标.结果:观察组创面细菌清除度、植皮前肉芽组织增长率、植皮前肉芽组织生长厚度、创面愈合率、创面愈合速率均优于对照组,组间差异有统计学意义(P<0.05).观察组创面愈合时间短于对照组,治疗总有效率高于对照组,组间差异有统计学意义(P<0.05).结论:VSD联合APG能有效促进糖尿病慢性难愈合创面患者肉芽组织生长、缩短创面愈合时间和提高创面植皮愈合质量,提高治疗效果.
Abstract
Objective:To analyze the application of continuous closed negative pressure drainage(VSD)combined with autologous platelet-rich plasma gel(APG)in skin grafting treatment of chronic refractory diabetic wounds.Methods:80 patients with chronic refractory diabetic wounds admitted to The third hospital of Inner Mongolia autonomous region from February 2021 to June 2022 were selected as the study objects.They were randomly divided into an observation group(n=37)and a control group(n=43).After debridement,the wound was covered by VSD in the control group.After debridement,APG was evenly applied on the wound in the observation group,and then VSD was applied to cover the wound.Skin grafting was performed when the wound of the two groups reached the condition of skin graft.Bacterial clearance before skin grafting,growth rate of granulation tissue before skin grafting,growth thickness of granulation tissue before skin grafting,wound healing rate,wound healing rate and curative effect indexes were compared between the two groups.Results:The bacterial clearance,growth rate of granulation tissue before skin grafting,growth thickness of granulation tissue before skin grafting,wound healing rate and wound healing rate of observation group were better than those of control group,and the difference was between the two groups statistically significant(P<0.05).The wound healing time of the observation group was shorter than that of the control group,and the total effective rate was higher,the difference was statistically significant(P<0.05).Conclusion:VSD combined with APG can effectively promote the growth of granulation tissue,shorten the time of wound healing,improve the healing quality of wound skin grafting and improve the therapeutic effect.
