协同护理管理辅助加速康复外科理念在老年胸腰椎骨折中的应用效果

摘要
目的:观察协同护理管理辅助加速康复外科理念在老年胸腰椎骨折经皮椎体成形术患者护理中的应用效果,分析其临床价值.方法:选择 2022 年1 月—2023 年 1 月在我院接受经皮椎体成形术治疗的 60 例老年胸腰椎骨折患者为研究对象,以电脑便利抽样将其随机分为对照组(n=30)和试验组(n=30).对照组患者围手术期施以常规护理干预,试验组患者围手术期施以协同护理管理辅助加速康复外科理念干预.结果:试验组患者手术操作耗时、住院治疗天数及术后首次离床活动时间均较对照组患者更短,术中出血总量较对照组患者更少,组间差异有统计学意义(P<0.05);出院时,2 组患者自我护理能力测定量表各个维度评分及总分均高于干预前,且试验组患者ESCA各个维度评分及总分均较对照组更高,差异有统计学意义(P<0.05);试验组患者围手术期并发症发生率低于对照组患者,差异有统计学意义(P<0.05);2 组患者VAS评分、JOA评分及ODI评分均较术前显著改善,且试验组患者VAS评分、ODI评分均低于对照组患者,JOA评分高于对照组患者,差异有统计学意义(P<0.05).结论:将协同护理管理辅助加速康复外科理念应用于老年胸腰椎骨折经皮椎体成形术患者围手术期的护理中,能够缩短患者手术操作耗时,可减轻患者术后应激反应,加快患者的恢复进程,提高患者自护能力,缓解术后疼痛,促进腰椎功能的恢复,并降低并发症的发生.
Abstract
Objective:To observe the application effect of the concept of collaborative nursing management assisted accelerated re-habilitation surgery in the nursing of elderly patients with thoracolumbar fracture undergoing percutaneous vertebroplasty,and to analyze its clinical value.Methods:A total of 60 elderly patients with thoracolumbra fracture who received percutaneous vertebroplasty in our hospital from January 2022 to January 2023 were selected as the study objects,and randomly divided into a control group(n=30)and an experimental group(n=30)by computer-facilitated sampling.The control group received routine nursing intervention during periopera-tive period,and the experimental group received collaborative nursing management assisted accelerated rehabilitation surgery concept intervention during perioperative period.Results:The operation time,hospitalization days and the first time to get out of bed after surgery in experimental group were shorter than those in control group,and the total amount of intraoperative bleeding was less than that in control group,and the difference between the two groups was statistically significant(P<0.05).At discharge,Exercise of Self-care Agency Scale score and total score of 2 groups were higher than before intervention,and ESCA score and total score of experimental group were higher than control group,and the difference was statistically significant(P<0.05).The incidence of perioperative complica-tions in experimental group was lower than that in control group,and the difference was statistically significant(P<0.05).VAS score,JOA score and ODI score of patients in 2 groups were significantly improved compared with those before surgery,and VAS score and ODI score of patients in experimental group were lower than those in control group,and JOA score was higher than that in control group,and the difference was statistically significant(P<0.05).Conclusion:The application of the concept of collaborative nursing management assisted accelerated rehabilitation surgery in the perioperative nursing of elderly patients with thoracolumbar fracture un-dergoing percutaneous vertebroplasty can shorten the operation time of patients,reduce the postoperative stress reaction of patients,ac-celerate the recovery process of patients,improve the self-care ability of patients,relieve postoperative pain,promote the recovery of lumbar function,and reduce the occurrence of complications.
