疼痛护理管理路径对肱骨近端骨折患者疼痛程度及肩关节功能恢复的影响

摘要
目的:探究疼痛护理管理路径对肱骨近端骨折患者疼痛程度及肩关节功能恢复的影响.方法:选取 2019 年 10 月—2020 年11 月我院收治的 104 例肱骨近端骨折患者为研究对象,按随机数字表法将其分为对照组和观察组,各 52 例.对照组采用常规护理干预,观察组采用疼痛护理管理路径干预,2 组均持续护理 3 个月.比较 2组患者疼痛程度、心理状态、肩关节功能恢复情况、睡眠质量、护理满意度.结果:护理前,2 组疼痛程度、心理状态、肩关节功能恢复情况、睡眠质量比较,组间差异无统计学意义(P>0.05);护理后,观察组疼痛视觉模拟评分、抑郁自评量表评分、焦虑自评量表评分均低于对照组,疼痛、日常生活活动能力、关节活动范围及肌力评分均高于对照组,匹兹堡睡眠质量指数量表评分低于对照组,组间差异有统计学意义(P<0.05);观察组护理满意度 96.15%高于对照组的 82.69%,差异有统计学意义(P<0.05).结论:疼痛护理管理路径能够减轻肱骨近端骨折疼痛、负性情绪,改善睡眠质量,促进肩关节功能快速恢复,有利于提高护理满意度.
Abstract
Objective:To explore the effect of pain nursing management pathway on pain degree and shoulder joint function recovery in patients with proximal humeral fractures.Methods:104 patients with proximal humeral fractures admitted to our hospital from October 2019 to November 2020 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 52 cases in each group.Patients in the control group received routine nursing intervention,while patients in the observation group received pain nursing management pathway intervention,both groups received continuous care for 3 months.The pain degree,psychological state,recovery of shoulder joint function,sleep quality and nursing satisfaction were compared between the two groups.Results:Before nursing,there was no significant difference in pain degree,psychological state,recovery of shoulder joint function and sleep quality between the two groups(P>0.05).After nursing,the scores of pain visual analogue scale,Self-rating Depression Scale and Self-rating Anxiety Scale in the observation group were lower than those in the control group,the scores of pain,activities of daily living,joint range of motion and muscle strength were higher than those in the control group,and the score of Pittsburgh Sleep Quality Index was lower than that in the control group,and the differences between the two groups were statistically significant(P<0.05).The nursing satisfaction of the observation group 96.15% was higher than 82.69% of the control group,and the difference was statistically significant(P<0.05).Conclusion:Pain nursing management pathway can relieve pain and negative emotions of proximal humeral fracture,improve sleep quality,promote the rapid recovery of shoulder joint function,and improve nursing satisfaction.
