膝关节外伤性骨挫伤患者行MR诊断及其临床意义分析

摘要
目的:探讨膝关节外伤性骨挫伤患者行MR诊断及其临床意义.方法:将 2019年 6月—2021年 6月建昌县人民医院收治的82例膝关节外伤性骨挫伤患者纳入研究.外伤发生当日行CT诊断,且在伤后1~3 d行MR诊断,以关节镜诊断结果作为"金标准",对CT与MR诊断的结果进行比较.结果:CT与MR诊断,均能够检测出骨折、膝关节积液、挫伤组织肿胀问题,MR诊断 10例骨折、12例软组织肿胀、15例积液,而CT诊断分别为 8、12、15例.且MR还能明确诊断出骨挫伤 34例,韧带损伤 6例,半月板损伤 5例,而CT诊断不能检出.MR诊断的准确率为 90.24%,CT诊断的准确率为71.95%,对比差异有统计学意义(P<0.05);MR不同分型检出率均高于CT诊断,但差异均无统计学意义(P>0.05).结论:MR诊断运用于膝关节外伤性骨挫伤诊断中,能够提供更为全面的诊治信息,提高诊断的准确率.
Abstract
Objective:To investigate the clinical significance of MR diagnosis in patients with traumatic bone contusion of knee joint.Methods:A total of 82 patients with traumatic bone contusion of knee joint admitted to Jianchang County People's Hospital from June 2019 to June 2021 were included in the study.CT diagnosis was performed on the day of trauma,and MR Diagnosis was performed 1~3 days after injury,the arthroscopic diagnosis was the"gold standard",the results of CT diagnosis and MR diagnosis were compared.Results:Both CT and MR diagnosis could detect fracture,knee joint effusion and contusion tissue swelling.MR diagnosis showed 10 cases of fracture,12 cases of soft tissue swelling and 15 cases of effusion,while CT diagnosis showed 8,12 and 15 cases respectively.In addition,34 cases of bone contusion,6 cases of ligament injury and 5 cases of meniscus injury were clearly diagnosed by MR,while CT diagnosis could not detect them.The accuracy of MR Diagnosis was 90.24%,and that of CT diagnosis was 71.95%,the difference was statistically significant(P<0.05).The detection rate of different types of MR Was higher than that of CT diagnosis,but the difference was not statistically significant(P>0.05).Conclusion:The application of MR diagnosis in the diagnosis of traumatic bone contusion of knee joint can provide more comprehensive diagnosis information and improve the accuracy of diagnosis.
