身痛逐瘀汤辅助椎间孔镜治疗血瘀气滞型腰椎间盘突出症急性期的效果观察
Observation on The Effect of Shendong Zhuyu Decoction Assisted Foraminal Mirror in The Treatment of Acute Stage of Lumbar Disc Herniation with Blood Stasis and Qi Stagnation

摘要
目的:探讨身痛逐瘀汤辅助椎间孔镜治疗血瘀气滞型腰椎间盘突出症(LDH)急性期的效果.方法:选取该院 2020年 1月—2020年 12月行椎间孔镜手术治疗的 60例血瘀气滞型LDH患者为研究对象,按随机数字表法将其分为对照组与观察组,各 30例.术后 1 d,对照组采用腰痹通胶囊治疗,观察组采用身痛逐瘀汤治疗.比较2组临床疗效、中医证候、疼痛情况、腰椎功能、血清细胞因子、安全性.结果:治疗4周后,观察组治疗总有效率为96.67%,显著高于对照组的80.00%,差异有统计学意义(P<0.05).观察组各个维度积分,均低于对照组,组间差异有统计学意义(P<0.05).观察组疼痛情况及腰椎功能均优于对照组,组间差异有统计学意义(P<0.05).观察组各细胞因子水平均明显低于对照组,组间差异有统计学意义(P<0.05).结论:身痛逐瘀汤辅助椎间孔镜治疗血瘀气滞型LDH急性期疗效显著,可有效抑制炎症反应,缓解患者临床症状和腰腿疼痛情况,改善腰椎功能,且用药安全,值得推广应用.
Abstract
Objective:To investigate the effect of Shendong Zhuyu Decoction assisted foraminal mirror on acute stage of lumbar disc herniation(LDH)with blood stasis and qi stagnation.Methods:Sixty patients with blood stasis and qi stagnation type LDH who underwent foraminal endoscopic surgery in our hospital from January 2020 to December 2020 were selected as the study objects,and were divided into control group and observation group according to random number table method,with 30 cases in each group.1 day after operation,the control group was treated with Waibitong capsule,and the observation group was treated with Shentong Zhuyu decoction.The clinical efficacy,TCM syndrome,pain,lumbar function,serum cytokines and safety of the two groups were compared.Results:After 4 weeks of treatment,the total effective rate of the observation group was 96.67%,which was significantly higher than that of the control group 80.00%,and the difference was statistically significant(P<0.05).The scores of all dimensions in the observation group were lower than those in the control group,and the differences between groups were statistically significant(P<0.05).The pain and lumbar function of the observation group were better than those of the control group,and the differences between the two groups were statistically significant(P<0.05).The levels of cytokines in the observation group were significantly lower than those in the control group,with statistical significance(P<0.05).Conclusion:Shantong Zhuyu Decoction assisted foraminal mirror in the treatment of blood stasis and qi stagnation type LDH in the acute stage has significant effect,can effectively inhibit inflammation,relieve patients'clinical symptoms and lumbar and leg pain,improve lumbar function,and is safe to use,worthy of promotion and application.
