摘要

目的:探讨超声引导下持续股神经阻滞的应用效果.方法:选择 2019年 6月—2021年 4月于本院接受全膝关节置换术治疗的 78例患者为研究对象,以就诊顺序的奇偶将其分为常规组和试验组,各 39例.常规组采用静脉自控镇痛,试验组采用超声引导下持续股神经阻滞镇痛,统计 2组应用效果.结果:术后 6、12、24及 48 h,试验组疼痛模拟评分法评分均低于常规组,组间差异有统计学意义(P<0.05).术后48 h,试验组C反应蛋白及白细胞介素-6均低于常规组,组间差异有统计学意义(P<0.05).试验组不良反应总发生率为 7.69%,低于常规组的 12.82%,差异有统计学意义(P<0.05).治疗后试验组经皮动脉血氧饱和度高于常规组,差异有统计学意义(P<0.05).结论:超声引导下持续股神经阻滞可有效缓解全膝关节置换术后疼痛,能够减轻患者炎症反应,安全性较高.

Abstract

Objective:To investigate the application effect of ultrasound-guided continuous femoral nerve block after total knee arthroplasty.Methods:78 patients who received total knee arthroplasty in our hospital from June 2019 to April 2021 were selected as the study objects,they were divided into a conventional group and an experimental group according to the odd-even sequence visit number,with 39 cases in each group.The conventional group was treated with intravenous controlled analgesia,the experimental group was treated with ultrasound-guided continuous femoral nerve block.Results:At 6,12,24 and 48 h after surgery,the scores of Pain Visual Analogue Scales in the experimental group were lower than those of the conventional group,the differences were statistically significant between the two groups(P<0.05).At 48 h after surgery,C-reactive protein and interleukin-6 in the experimental groups were lower than those in the conventional group,the differences between groups were statistically significant(P<0.05).The total incidence of adverse reactions in experimental group was 7.69%,which was lower than 12.82%in conventional group,and the difference was statistically significant(P<0.05).After treatment,the percutaneous arterial oxygen saturation of experimental group was higher than that of conventional group,and the difference was statistically significant(P<0.05).Conclusion:Ultrasound-guided continuous femoral nerve block can effectively relieve pain after total knee arthroplasty,and can reduce the inflammatory response of patients with high safety.
