ERAS理念下老年髋部骨折患者围术期禁食禁饮方案的应用效果观察

摘要
目的:在加速康复外科理念下,实施老年髋部骨折患者围术期禁食禁饮管理方案,并探讨该方案的临床应用效果.方法:选择我院骨科 2021年 6月—2023年 3月收治的 48例髋部骨折手术患者为研究对象.按不同禁食禁饮管理方案分为观察组与对照组,各 24例.观察组采用ERAS理念下围手术期禁食禁饮管理,对照组采用传统饮食管理.结果:观察组术前、术后禁食禁饮时间均短于对照组,组间差异有统计学意义(P＜0.05).2组术后恶心呕吐发生率水平比较,差异无统计学意义(P＞0.05);观察组术前饥饿发生率、术后腹胀、便秘发生率均明显低于对照组,组间差异有统计学意义(P＜0.05).结论:遵循ERAS理念构建的老年髋部骨折围术期禁食禁饮管理方案安全可行,值得临床借鉴实施.
Abstract
Objective:To implement the perioperative fasting and water deprivation management program for elderly patients with hip fracture under the concept of enhanced recovery after surgery(ERAS),and to explore the clinical application effect of the program.Methods:48 patients with hip fracture surgery admitted to the Department of Orthopedics of our hospital from June 2021 to March 2023 were selected as the research objects.According to different fasting and water deprivation management programs,they were divided into observation group and control group,24 cases in each group.The observation group received perioperative fasting and water deprivation management under the concept of ERAS.The control group was treated with traditional diet management.Results:The preoperative and postoperative fasting and water deprivation time in the observation group were shorter than those in the control group,and the difference between the two groups was statistically significant(P＜0.05).There was no significant difference in the incidence of postoperative nausea and vomiting between the two groups(P＞0.05).The incidence of preoperative hunger,postoperative abdominal distension and constipation in the observation group were significantly lower than those in the control group,and the differences between the two groups were statistically significant(P＜0.05).Conclusion:The ERAS management program for perioperative fasting and water deprivation in elderly patients with hip fracture is safe and feasible,which is worthy of clinical reference and implementation.
