踝关节全修复联合外敷芒硝对踝关节骨折术后功能恢复的影响

摘要
目的:探讨踝关节全修复联合外敷芒硝对踝关节骨折术后功能恢复的影响.方法:选择 2019年 3月—2023年 1月我院收治的 70例踝关节骨折患者作为研究对象,根据随机数字表法将所有患者分成对照组和观察组,每组35例.所有患者均采用踝关节全修复治疗,观察组在踝关节全修复术后联用外敷芒硝治疗.采用美国矫形足踝协会(AOFAS)评分对患者踝关节功能改善情况进行评价,并测量患者踝关节背伸和跖屈活动度,比较 2组患者术后 1、3、6 d健肢与患肢周径差,术后 1、6 d疼痛视觉模拟(VAS)评分,以及治疗后 2组患者的血清D-二聚体水平.结果:治疗前 2组患者的AOFAS评分、踝关节背伸、踝关节跖屈活动度比较,组间差异无统计学意义(P>0.05),治疗后2组患者的AOFAS评分、踝关节背伸及跖屈活动度均较治疗前显著提升,差异有统计学意义(P<0.05),但组间比较差异无统计学意义(P>0.05).2组患者术后 1 d健肢与患肢周径差比较,差异无统计学意义(P>0.05),2组患者术后 3、6 d健肢与患肢周径差均逐渐减少,并且观察组患者术后3 d、6 d健肢与患肢周径差均低于对照组,组间差异有统计学意义(P<0.05).2组患者术后1 d VAS评分比较,差异无统计学意义(P>0.05),术后 6 d观察组VAS评分低于对照组,差异有统计学意义(P<0.05).治疗后观察组患者的血清D-二聚体水平低于对照组,差异有统计学意义(P<0.05).结论:踝关节骨折患者接受踝关节全修复治疗,能有效改善患者踝关节各项功能,若术后联合外敷芒硝干预,对缓解术后肿胀及疼痛有显著效果,也能显著促进其凝血指标的改善.
Abstract
Objective:To investigate the effect of total ankle joint repair combined with Glauber's salt on functional recovery after ankle fracture.Methods:70 patients with ankle fracture admitted in our hospital from March 2019 to January 2023 were selected as research objects,and all patients were divided into a control group and an observation group according to random number table method,with 35 cases in each group.All patients were treated with total ankle joint repair,and the observation group was treated with external Glauber's salt after total ankle joint repair treatment.The improvement of the ankle joint function of the patients was evaluated by the American Orthopaedic Foot and Ankle Association(AOFAS)score,and the dorsal extension motion and plantar-flexor motion of the patients were measured.The difference of the circumference diameter between the healthy limb and the affected limb on day 1,3 and 6 of Postoperation,the pain Visual Analog Scale(VAS)score at 1 and 6 d of Postoperation,and the serum D-dimer level after treatment were compared between the two groups.Results:There were no statistically significant differences in AOFAS score,ankle dorsal-extension and ankle plantar flexion motion between the two groups before treatment(P>0.05).After treatment,AOFAS score,ankle dorsal-extension and plantar flexion motion between the two groups were significantly improved(P<0.05),and there were no statistically significant differences between the two groups(P>0.05).There were no statistical significance in the differences of the circumference diameter between the healthy limb and the affected limb in the two groups 1 d after surgery(P>0.05),and the difference of the circumference diameter between the healthy limb and the affected limb in the 2 groups was gradually reduced 3 and 6 d after surgery,and the difference between the healthy limb and the affected limb on 3 and 6 d after surgery in the observation group was lower than that in the control group,with statistical significance between the two groups(P<0.05).There was no significant difference in Postoperative VAS score between the two groups 1 d after surgery,and VAS score in the observation group was lower than that in the control group 6 d after surgery,with statistically significant differences between the two groups(P<0.05).After treatment,the serum D-dimer level of observation group was lower than that of control group,and the difference was statistically significant(P<0.05).Conclusion:The total repair treatment of ankle joint can effectively improve the various functions of ankle joint in patients with ankle fracture.If the intervention combined with external application of Glauber's salt after surgery,it has a significant effect on alleviating postoperative swelling and pain,and can also significantly promote the improvement of coagulation index.
