股骨近端防旋髓内钉与动力髋螺钉内固定治疗股骨粗隆间骨折的效果比较

摘要
目的:对股骨粗隆间骨折治疗中股骨近端防旋髓内钉(PFNA)与动力髋螺钉(DHS)内固定治疗的效果进行比较.方法:选取 2019年 7月—2021年 7月我院收治的72例股骨粗隆间骨折患者为研究对象,按照掷硬币法分为参照组和试验组,各 36例.参照组采用DHS内固定治疗,试验组采用PFNA治疗,对比 2组患者的手术指标、手术疗效、并发症发生情况、髋关节功能、炎性因子水平.结果:试验组患者术中出血量少于参照组,手术时间、骨折愈合时间、住院时间及下地负重时间均短于参照组,治疗优良率高于参照组,术后并发症发生率低于参照组,组间差异有统计学意义(P<0.05);术后 6个月,2组髋关节活动度、运动、功能性活动评分均高于术前,且试验组高于参照组,差异有统计学意义(P<0.05);术后 1 d,试验组TNF-α、IL-6、IL-10 水平低于参照组,组间差异有统计学意义(P<0.05).结论:在股骨粗隆间骨折治疗中,PFNA治疗效果优于DHS内固定,可减少患者术中出血量,降低炎性因子水平,缩短患者骨折愈合时间,促进患者髋关节功能的改善.
Abstract
Objective:To compare of the effects of proximal femoral nail antirotation(PFNA)and dynamic hip screw(DHS)internal fixation in the treatment of intertrochanteric fractures of femur.Methods:72 patients with intertrochanteric fractures of femur admitted to our hospital from July 2019 to July 2021 were selected as the research subjects,divided into a reference group and an experimental group according to the coin toss method,with 36 cases in each group.The reference group received DHS internal fixation treatment,while the experimental group received PFNA treatment.The surgical indicators,surgical efficacy,occurrence of complications,hip joint function,and levels of inflammatory factors between two groups of patients were compared.Results:The experimental group had less intraoperative bleeding than the control group,shorter surgical time,fracture healing time,hospital stay,and weight-bearing time compared to the control group.The excellent and good treatment rate was higher than that of the control group,and the incidence of postoperative complications was lower than that of the control group,the differences between groups were statistically significant(P<0.05);6 months after surgery,the hip joint range of motion,movement,and functional activity scores in both groups were higher than before surgery,and the experimental group was higher than the reference group,the differences was statistically significant(P<0.05);1 day after surgery,in the experimental group TNF-α、IL-6 and IL-10 levels are lower than the reference group,the differences between groups were statistically significant(P<0.05).Conclusion:In the treatment of intertrochanteric fractures of femur,PFNA has a better therapeutic effect than DHS internal fixation.It can reduce intraoperative bleeding,lower inflammatory factor levels,shorten fracture healing time,and promote the improvement of hip joint function in patients.
